不同年龄儿童感染肺炎支原体体液免疫的变化分析 被引量:23

摘要 目的:通过对不同年龄肺炎支原体肺炎(mycoplasma pneumoniae pneumonia,MPP)患儿的体液免疫结果进行分析,探讨肺炎支原体(mycoplasma pneumoniae,MP)感染的发病机制,为MPP患儿体液免疫报告的解读及临床诊治提供参考。方法:352例MPP患儿纳入研究,使用荧光定量PCR检测MPP患儿痰液中的MP-DNA拷贝数,德灵-BN2检测患儿血清IgG、IgA、IgM、C3、C4含量,同时结合细菌培养及病原学9项检测排除其他病原体感染。分析MPP组各年龄段之间及MPP组与对照组之间体液免疫水平的差异。结果:MPP患儿的IgG、IgA、IgM、C3、C4水平均高于对照组(P<0.05),MPP患儿女性C4含量明显低于男性(P<0.05),不同年龄段MPP患儿的IgG、IgA、IgM、C3含量差异有统计学意义(P<0.05),各年龄段MPP患儿体液免疫水平高于对照组。结论:MPP患儿各年龄段体液免疫水平存在差异且均高于对照组,临床在对患儿的体液免疫进行评估时须结合同年龄段儿童的正常参考水平才能正确分析MPP患儿体液免疫状态。 Objective: To investigate the differences of humoral immunity function among children with mycoplasma pneumoniae pneumonia( MPP) in different ages,and provide guidance for clinical interpretation of humoral immunity report. Methods: 352 children with MPP were involved in this study,real-time PCR was used for children sputum MP-DNA testing,serum concentrations of IgG,IgA,IgM,C3,C4 were detected by BN2.Bacteria culture and pathogen detection of respiratory tract were used to eliminate other pathogen infections. The differences among different ages of MPP children and the differences between MPP children and healthy control were analyzed. Results: Serum concentrations of IgG,IgA,IgM,C3,C4 were higher in MPP children compared to healthy control( P〈0. 05). Serum concentration of C4 was higher in male MPP children( P〈0. 05). Serum concentrations of IgG,IgA,IgM,C3 were significantly different( P〈0. 05) among different ages of MPP children and were higher than those in healthy control in each age group. Conclusion: MPP children have a higher level of humoral immunity in each age group,and clinical evaluation of children humoral immunity function should be adjusted according to children ages.
