慢性肾小球肾炎慢病管理71例临床观察

Clinical Observation of Chronic Disease Management on Chronic Glomerulonephritis for 71 Cases

目的探索出有效的慢性肾小球肾炎慢性病管理门诊模式。方法本研究共收集2013年2月—2014年2月长春中医药大学附属医院肾内科门诊及住院患者,根据严格的纳入和排除标准,选择71例慢性肾小球肾炎患者,随机分为实验组37例、对照组34例。实验组在基础治疗同时加慢病干预措施,对照组:单纯基础治疗。疗程6个月。根据疗效评定标准评定疗效,观察24小时尿蛋白定量,尿沉渣中尿红细胞计数,肾功能,血压达标情况。结果疗效评估有效率、24小时尿蛋白定量、尿红细胞计数、肾功能、血压达标率治疗前后明显改善,两组比较有统计学意义(P<0.05)。结论运用中西医方法,配合慢病管理手段,可改善慢性肾小球肾炎患者生活质量,减轻患者血尿、蛋白尿,改善肾功能,协助血压达标,并可延缓其进入终末期肾病。

Objective To explore an effective outpatient service model on chronic disease management of chronic glomerular nephritis.Methods This study collected inpatients and outpatients from February 2013 to February 2014 in Department of Nephrology of the First Affiliated Hospital of Changchun University of Chinese Medicine, according to strict exclusion criteria, 71 cases of patients with chronic glomerulo nephritis were randomly divided into experimental group of 37 cases and control group of 34 cases. The experi-mental group was treated chronic disease intervention measures and foundation treatment. The control group was treated with pure foundation treatment. After treatment of 6 months, according to the curative effect evaluation criteria, the effective rate, 24 hours uri-nary protein, urine red blood cell count, renal function and blood pressure were observed. Results The effective rate of curative effect evaluation, 24-hour urine protein, urine red blood cell count, renal function, blood pressure after treatment obviously improved, and the difference was statistically significant（P〈0.05）. Conclusion Using the method of Chinese and Western medicine combined with chronic disease management can improve the quality of life in patients with chronic glomerulonephritis, relieve hematuria and protein-uria, improve renal function, help the blood pressure control, and can delay end-stage renal disease.
