1例心功能不全终末期伴多浆膜腔积液患者的药学监护

Pharmaceutical care on patients with cardiac insufficiency at the end stage and multiple serous cavity effusion: A case report

目的:探讨临床药师在心力衰竭合并腹腔积液患者治疗过程中的作用。方法:临床药师参与1例心力衰竭合并腹腔积液患者的全部治疗过程。在患者的治疗过程中,临床药师运用药学知识为医生提供建议,对患者进行药学监护,提供用药教育。结果:医生采纳药师建议,经过治疗后,患者心衰症状缓解,腹腔积液减少,病情好转出院。结论:临床药师可以协助医生完善个体化治疗方案,同时为患者提供药学服务,有利于提高临床治疗的合理性、安全性、有效性。

Objective：To explore the role of clinical pharmacists in treatment of a patient with heart failure and seroperitoneum. Methods： Clinical pharmacists participated in the whole process of medical treatment, provided some advice to the doctors,and delivered medication monitoring and drug education to the patients.Results： Doctors followed the advice of pharmacists. With a series of treatment,the patient’s symptoms of heart failure relieved and abdominal effusion decreased. Finally the patient discharged.Conclusion：Clinical pharmacists can help doctors make the individualized therapy plan and provide patients with pharmaceutical services.It is beneifcial to improve the rationality,safety and effectivity in clinical treatment.
