CT灌注成像结合动态增强在孤立性肺结节鉴别诊断中的应用价值 被引量:7

摘要 目的分析孤立性肺结节(SPN)鉴别诊断中CT灌注成像结合动态增强扫描的应用价值。方法随机抽取我院收治的75例SPN患者为研究对象,给予同层动态CT增强扫描,根据扫描结果,建立时间-密度曲线(TDC),并进行统计、分析。结果 (1)与炎性结节、小肺癌相比,结核瘤的BV、BF、PS均显著较低(P<0.05);(2)实施增强扫描后,与结核瘤相比,炎性结节和小肺癌的CT值均明显较高(P<0.05);(3)实施增强扫描后,结核瘤组表现平坦,炎性结节组呈持续上升趋势,小肺癌组呈动态平衡状态。结论在SPN鉴别诊断中,应用CT灌注成像结合动态增强扫描,可提高诊断准确率。 Objective To analyze the application value of CT perfusion imaging combined with dynamic enhancement in the dif-ferential diagnosis of solitary pulmonary nodules ( SPN) . Methods 75 cases of SPN patients admitted to our hospital were randomly selected as the research objects and were given the same layer dynamic CT enhanced scanning. According to the results of the scan, time density curve (TDC) was established, and statistically analyzed. Results (1) Compared with inflammatory nodules and small cell lung carcinoma, tuberculoma BV, BF and PS were significantly lower (P〈0. 05). (2) After the enhanced scanning, compared with tuberculoma, inflammatory nodules and small lung cancer CT values were significantly higher ( P〈0. 05 ) . ( 3 ) After the en-hanced scanning, the tuberculosis group had flat manifestation, the inflammatory nodule group showed a rising trend, and the small lung cancer group was in a state of dynamic equilibrium. Conclusion CT perfusion imaging combined with dynamic enhanced scan can improve the diagnostic accuracy in the differential diagnosis of SPN.
