个案管理模式在安置永久性起搏器患者中的应用

Application of Case Management Model on Patients with Permanent Pacemaker

目的探讨个案管理模式在安置起搏器患者中的应用。方法回顾性选择2012年1月份至2013年5月份在锦州医科大学附属第一医院心内科住院行永久起搏器安置术后的患者60例作为对照组,2013年6月份至2014年12月份在辽宁医学院附属第一医院心内科住院行永久起搏器安置术后的患者60例作为干预组,对照组按护理常规,个案管理组通过入院期间的个案管理干预,以全方位系统的评估患者,知识宣教及技能引导,特别是咨询和随访出院后的干预。以提高患者的满意度和生活质量。结果对照组与干预组在医疗费用、出院患者满意度及生活质量的差异均具有统计学意义(P<0.05,P<0.01,P<0.05)。结论个案管理模式在安置起搏器患者中的应用可以缩减医疗支出,有效提高患者满意度、生活质量。

Objective To explore the application of case management model on patients with pacemaker. Methods 60 cases with permanent pacemaker from department of internal medicine-cardiovascular of the first affiliated hospital of Liaoning medical uni-versity from January 2012 to May 2013 were retrospectively selected as the control group. 60 cases with permanent pacemaker from de-partment of internal medicine-cardiovascular of the first affiliated hospital of Liaoning medical university from June 2013 to December 2014 were selected as the intervention group. Patients in the control group were treated according to nursing routine. Through case management intervention during hospitalization, patients in the intervention group were evaluated by comprehensive system and given knowledge education and skill guidance, especially counseling and intervention after discharge, so as to improve patients’ satisfaction and quality of life. Results Medical costs, satisfaction of discharged patients and quality of life between the control group and the in-tervention group were statistically significant （P〈0. 05, P〈0. 01, P〈0. 05）. Conclusion The application of case management mod-el on patients with pacemaker can reduce medical expenses and effectively improve patients’ satisfaction and quality of life.
