摘要
目的探讨2型糖尿病合并冠心病及高血压患者的血栓前状态及细胞色素P4502C19(CYP2C19)、乙醛脱氢酶2(ALDH2)基因(GLu504Lys)多态性位点的关系。方法选择新疆医科大学第一附属医院住院治疗的无亲缘关系的2型糖尿病合并冠心病和高血压患者90例(病例组),单纯冠心病患者117例(冠心病组),及新疆医科大学第一附属医院体检中心体检的健康人129例(对照组)。采用流式细胞仪法、酶联免疫吸附测定(ELISA)法、电阻抗法等检测各组人群血栓前状态指标及各生化指标,采用DNA微阵列芯片法将分析细胞色素P4502C19(CYP2C19)和乙醛脱氢酶2(ALDH2)基因GLu504Lys多态性位点在3组中分布的差异。结果 3组纤维蛋白原(FIB)、糖化血红蛋白(Hb A1c)、葡萄糖(GLU)、体质质数(BMI)及CYP2C19基因和ALDH2基因(GLu504Lys)多态性位点在3个组之间,差异均有统计学意义(P<0.05);血红蛋白、低密度蛋白胆固醇(Hb、LDL-C)在病例组和对照组差异有统计学意义(P<0.05),而冠心病组与其他两个组差异无统计学意义(P>0.05);总胆固醇、高密度脂蛋白胆固醇(TC、HDL-C)在病例组与冠心病组和健康对照组差异均有统计学意义(均P<0.05),但病例组与冠心病组两者之间差异无统计学意义(P>0.05)。其他指标在3个组之间差异均无统计学意义(均P>0.05);结论与冠心病组和健康对照组比较,2型糖尿病合并冠心病及高血压组患者存在血栓前状态。Hb A1c、BMI、CYP2C19基因及ALDH2基因GLu504Lys多态性位点是2型糖尿病合并冠心病及高血压组的危险因素;BMI是单纯冠心病组的危险因素。
[Objective]To explore the correlation between prethrombotic state,Cytochrome P4502C19(CYP2C19)and Aldehyde dehydrogenase2 genetic(Glu504Lys)polymorphism loci among patients with type 2 diabetes complicated by coronary heart disease and hypertension.[Methods]A total of 90 cases of type 2 diabetes complicated by coronary heart disease and hypertension patients hospitalized in the First Affiliated Hospital of Xinjiang Medical University were selected as case group,117 cases of coronary heart disease patients were as coronary heart disease group and 129 healthy people taking health examination in physical examination center were as control group. The flow cytometry,ELISA and electronic resistance methods were adopted to detect the prethrombotic state and biochemical indicators. The DNA microarray chip method was adopted to analyze the distribution differences of CYP2C19 and ALDH2 genetic(Glu504Lys)polymorphism loci in three groups [Results]The differences of fibrinogen(FIB),glycosylated hemoglobin(Hb A1c),glucose(GLU),BMI,CYP2C19 gene and ALDH2 gene(Glu504Lys)polymorphism loci were all statistically significant in three groups(all P〈0.05). The differences of Hb and low density lipoprotein(LDL-C)were statistically significant in case group and control group(P〈0.05),but were not statistically significant between coronary heart disease group and other two groups(P〈0.05). The differences of total cholesterol(TC),high-density lipoprotein(HDL-C)were statistically significant in three groups,but were not statistically significant between case group and coronary heart disease group(P〈0.05). The differences of other indicators were not statistically significant in three groups(P〈0.05).[Conclusion]Compared with coronary heart disease group and control group,there is prethrombotic state in patients with type 2 diabetes complicated by coronary heart disease and hypertension.The Hb A1 c,BMI,CYP2C19 gene and ALDH2 gene(Glu504Lys) polymorphism loci are the risk factors of type 2 diabetes complicated by coronary heart disease and hypertension group. BMI is the risk factor of the coronary heart disease group.
