EML4-ALK融合基因与EGFR基因突变在肺腺癌发生感染患者中的表达研究 被引量:4

摘要 目的探讨EML4-ALK融合基因与EGFR基因突变在肺腺癌发生感染患者中的表达,为指导临床治疗提供依据。方法选取医院2012年1月-2014年12月50例肺腺癌发生感染患者作为研究对象,采取ARMS法测定组织中EGFR基因突变,FISH法测定组织中EML4-ALK融合基因状况,并分析其与临床特征关系。结果50例肺腺癌发生感染患者病灶组织中4例EML4-ALK融合基因表达阳性,阳性率为8.0%,32例EGFR基因突变,突变率为64.0%;女性、年龄<60岁、吸烟指数<400支/年的肺腺癌发生感染患者病灶组织中EGFR基因突变率较高(P<0.05);EGFR基因突变与肺腺癌发生感染患者的临床分期和分化程度以及淋巴转移无相关性,差异无统计学意义;EML4-ALK融合基因表达与EGFR基因突变在肺腺癌发生感染患者中呈现负相关性,差异有统计学意义(r=-0.517,P<0.05)。结论肺腺癌发生感染患者中EML4-ALK融合基因呈现低表达,而EGFR基因突变呈现高表达,且两者在临床中呈现负相关性,对肺腺癌发生感染的临床治疗具有一定的指导意义。 OBJECTIVE To investigate the EML4-ALK fusion gene expression and EGFR gene mutation in infected patients with lung adenocarcinoma,so as to provide the basis for the clinical treatment.METHODS A total of 50 cases of infected patients with lung adenocarcinoma in the hospital from Jan.2012 to Dec.2014 were selected as research subjects.EGFR gene mutation was detected by ARMS assay method,tissue EML4-ALK fusion gene status were measured by FISH method,and their relationships with clinical characteristics were analyzed.RESULTS A total of 4cases of EML4-ALK fusion gene expression occurred in 50 cases of lung adenocarcinoma patients,with the positive rate of 8.0%,and 32 cases of EGFR gene mutation occurred,with the mutation rate of 64.0%.Lung infection in patients of female,age 〈60years,and smoking index 〈400cigarettes/year had significantly higher EGFR gene mutation rate(P〈0.05).EGFR mutations stage and extent of infection in patients with lung cancer clinical differentiation and lymph node metastasis of infected patients with lung adenocarcinoma had no correlation with EGFR gene mutation,and the difference was not significant.EML4-ALK fusion gene expression and EGFR gene mutation present had negative correlation in infected lung adenocarcinoma patients(r=-0.517,P〈0.05).CONCLUSIONEML4-ALK fusion gene exhibits low expression,and EGFR mutation exhibits high expression in infected patients with lung adenocarcinoma,and they have negative correlation in clinical,which will provide some guidance for treatment of infected patients with lung adenocarcinoma.
