消化性溃疡出血患者幽门螺杆菌感染的调查

Investigation of HP infection in patients with peptic ulcer bleeding

目的探讨消化性溃疡出血(PUB)患者的胃镜检查结果及幽门螺杆菌(Hp)感染情况,以期为PUB患者的预防及治疗提供帮助。方法选取2011年10月-2015年5月在医院诊治的150例PUB患者临床资料,根据是否Hp感染将其分为Hp阴性组41例、阳性组109例,另选取同期的150例消化性溃疡非出血患者作为对照组,其中Hp阴性组48例、Hp阳性组102例,分析PUB患者胃镜检查结果,比较PUB患者与消化性溃疡非出血患者的Hp感染,数据均采用SPSS 20.0统计软件进行处理。结果 PUB并Hp阴性组患者黑便25例、PUB并Hp阳性组黑便67例,黑便分布率分别为60.9%、61.5%,两组比较差异无统计学意义;PUB并Hp阴性组与PUB并阳性组患者均以十二指肠溃疡分布率最高,均>70.0%,Hp阳性组复合溃疡分布率略高于Hp阴性组,且两组比较差异无统计学意义;PUB患者Hp感染率72.7%,与非出血组感染率68.0%相似,两组间差异无统计学意义。结论 PUB患者出血类型为黑便、呕血、黑便与呕血;胃镜表现为十二指肠溃疡、胃溃疡、复合溃疡;PUB与未出血患者的Hp感染率比较,差异无统计学意义。

OBJECTIVE To discuss gastroscopy results and Helicobacter pylori（HP）infection situation in patients with peptic ulcer bleeding（PUB）,so as to provide supports for prevention and treatment of patients with PUB.METHODS The clinical data of 150 patients with PUB diagnosed and treated in our hospital from Oct.2011 to May 2015 were selected.They were divided into HP negative group（n=41）and HP positive group（n=109）according to the situation of HP infection.Another 150 patients with peptic ulcer without bleeding were selected as control group,including HP negative group of 48 cases and HP positive patients of 102 cases.The gastroscopy results of patients with PUB were analyzed,and the HP infection conditions were compared between the two groups.The data were analyzed by SPSS 20.0.RESULTS The melena occurred in 25 cases in HP negative group and 67 cases in HP positive group of patients with PUB,with the melena rate of 60.9% and 61.5%,with no significant difference.The duodenal ulcer occurred mostly in both HP negative and positive group of patients with PUB,with both duodenal ulcer rates〉70.0%.The complex ulcer rate of HP positive group was a little higher than that of HP negative group.The HP infection rates of patients with PUB and peptic ulcer without bleeding were 72.7% and 68.0%,and there was no significant difference between the two groups.CONCLUSION The bleeding ways of patients with PUB were melena,hematemesis,melena with hematemesis.The gastroscope results were duodenal ulcer,gastric ulcer,complex ulcer.The HP infection rates of patients with PUB and peptic ulcer without bleeding had no significant difference.
