CT引导下经皮穿刺活检技术在脊柱感染病变诊断中的应用研究 被引量:13

摘要 目的探讨CT引导下经皮穿刺活检在脊柱感染病变诊断中的应用价值,为临床诊断提供依据。方法回顾性分析2011年6月-2015年6月医院收治的在CT引导下经皮穿刺活检技术诊断的脊柱病变72例患者临床资料,观察穿刺活检术的病理结果,对穿刺活检的准确性、安全性进行分析。结果 72例患者中71例获得满意标本,占98.6%;72例成功活检患者中均未发生大血管损伤、感染、截瘫神经损伤等并发症;72例均有手术病理结果,其中穿刺活检结果与手术结果相符的67例,穿刺活检的准确率为93.1%;各脊柱感染病变诊断准确率均较高,其中溶骨性感染病变诊断准确率为98.1%,较成骨性感染及混合型感染病变诊断准确率80.0%明显升高,差异有统计学意义(P<0.05)。结论 CT引导下经皮穿刺活检技术具有安全、有效、微创、准确率高等优势,是确诊脊柱感染病变的有效方法,在脊柱感染病变诊断中具有非常高的应用价值。 OBJECTIVE To discuss the application value of CT guided percutaneous biopsy in the diagnosis of spinal infection disease,so as to provide the basis for clinical diagnosis.METHODS A total of 72 cases of clinical data under CT guided percutaneous biopsy of the spinal lesions were retrospectively analyzed in our hospital from Jun.2011 to Jun.2015.The pathological results of biopsy were observed,and the accuracy and security of the biopsy were analyzed.RESULTS 72 cases of patients A total of 71 cases got satisfactory materials in 72 patients,accounting for 98.6%.Complications such as vascular injury,infection,and nerve injury with paraplegia were not occurred in 72 cases of successful biopsy.Of surgical pathology confirmed in 72 patients as a result,there were 67 cases had the biopsy results according with the surgical results,with the biopsy accuracy of 93.1%.The diagnosis accuracy of spinal infection disease was high.The diagnosis accuracy of soluble osseous infection disease was98.1%,significantly higher than that of osseous infection and mixed infection disease(80.0%)(P〈0.05).CONCLUSIONCT guided percutaneous biopsy is safe,effective,minimally invasive and accurate,which is an effective method for diagnosis of spinal infection disease,and has very high application value in spinal infection disease diagnosis.
