预防血液透析患者医院感染预见性护理的应用效果分析

Application effect of nursing care in the prevention of nosocomial infections in in hemodialysis patients

目的探讨预见性护理应用于预防糖尿病肾病血液透析患者医院感染的临床效果,为降低糖尿病肾病血液透析患者医院感染率提供依据。方法通过分析2014年4月-2015年2月在医院进行治疗的186例糖尿病肾病血液透析患者临床资料,随机分为观察组和对照组,每组各93例,观察组患者给予预见性护理,对照组患者给予常规护理,统计两组患者医院感染率,并分析导致患者发生医院感染的相关因素,总结护理经验。结果治疗过程中,观察组患者发生医院感染45例,感染率为48.39%,对照组患者发生感染55例,感染率为59.14%,两组患者感染率比较差异有统计学意义(P<0.05);100例感染患者感染部位主要为血液占49.00%,其次为呼吸道占39.00%;根据患者是否发生医院感染分为感染组和未感染组,结果显示,发生感染组患者年龄、周透析次数、血清红蛋白及血清白蛋白同未感染组患者比较,差异有统计学意义(P<0.05)。结论采用预见性护理干预可在一定程度上降低糖尿病肾病血液透析患者医院感染率。

OBJECTIVE To investigate the clinical effect of nursing care in the prevention of nosocomial infections in patients with diabetic nephropathy underwent blood dialysis,so as to provide basis for reducing the incidence of nosocomial infections in patients with diabetic nephropathy underwent blood dialysis.METHODS The clinical data of 186 diabetic nephropathy patients underwent dialysis from Apr.2014 to Feb.2015 were analyzed and randomly divided into control group and observation group,with 93 patients in each group.Observation group was given nursing care,while control group was treated with routine care.The incidences of nosocomial infections in the two groups were calculated,clinical factors leading to the hospital infections were analyzed,and nursing experience was summarized.RESULTS Totally 45 cases of nosocomial infections were observed in observation group during treatment,infection rate was 48.39%.In control group,there were 55 cases of infections,and infection rate was59.14%.The difference in infection rates was significant（P〈0.05）.Among the 100 infected patients,the major infection site was blood passage（49.00%）,and then respiratory tract（39.00%）.Patients were divided into infected group and non-infected group based on nosocomial infections.According to the results,the four indicators age group,the number of weeks of dialysis,serum hemoglobin and serum albumin of infected group were significantly different from non-infected group（P〈0.05）.CONCLUSIONTo some extent,nursing care intervention can reduce the incidence of nosocomial infections in diabetic nephropathy underwent hemodialysis.
