腹腔镜和开腹右半结肠癌根治术临床对比研究

Clinical comparison of laparoscopy and open radical resection for right colon cancer

目的探讨腹腔镜和开腹右半结肠癌根治术的有效性和安全性，为临床治疗提供参考依据。方法回顾性分析81例结肠癌患者的临床资料，根据不同术式分为腹腔镜组（观察组，n=39）和开腹组（对照组，n=42）。比较两组患者的围术期情况、术后并发症发生率及近期预后。结果腹腔镜组术中失血量、切口长度、排气时间、进食时间、术后住院时间低于对照组（P〈0．05）。两组拔管时间、住院费用、手术时间、淋巴结清扫数目、术后并发症发生率及1、2年生存率、复发率比较差异未见统计学意义（P〉0．05）。结论腹腔镜右半结肠癌根治术近期预后与开腹根治术接近，且促进了患者早期康复，值得临床重视。

Objective To investigate the effectiveness and safety of laparoscopic and open radical resection for fight colon cancer, in order to provide reference for clinical treatment. Methods The clinical data of 81 cases of colon cancer were retrospectively analyzed, according to the different surgical methods, the patients were divided into laparoscopy group （n = 39 ） and open group （n = 42）. The perioperative, postoperative complications and short-term prognosis were compared between the two groups. Results The amount of blood loss, the length of the incision, the length of the incision, the time of eating, the time of eating was less in the observation group than those in the control group （P 〈 0. 05 ）. There was no significant difference between the two groups in the time of extubation, the cost of hospitalization, the operation time, the number of lymph node dissection, the incidence of postoperative complications, the survival rate or the recurrence rate of 1, 2 years （ P 〉 0. 05 ）. Conclusions The short-term prognosis of laparoseopic radical operation is close to open radical operation for fight colon cancer, and promote early rehabilitation of patients, it is worthy of attention.
