血清胱抑素C和尿微量白蛋白/肌酐比值在糖尿病肾病早期临床诊断 被引量:8

Serum cystatin C and urinary albumin/creatinine ratio in the diagnosis of early diabetic nephropathy

摘要 目的分析胱抑素c(CysC)和尿微量白蛋白(U—mAlb)/肌酐(cr)比值对糖尿病肾病早期损害的临床意义。方法选择2014年3月至2015年4月确诊为糖尿病患者95例,根据尿白蛋白排泄率结果分为正常尿蛋白组(A组)44例,早期DN组(B组)51例,选择同期健康体检者72例为对照组。应用Benckman全自动生化分析仪测定各组血清中CysC、尿素氮、Cr和尿微量白蛋白/肌酐比值含量。结果2型DM早期’肾损害,血清中Cy-sc浓度变化和尿微量白蛋白/肌酐比值含量高于正常对照组(P〈0.01),B组血清中CysC和尿微量白蛋白/肌酐比值含量高于A组,差异有统计学意义(P〈0.05)。结论CysC和U—mAlb/Cr可作为诊断糖尿病肾病早期肾损害敏感可靠指标。联合测定可提高检测阳性率,对糖尿病肾病早期肾损害的临床诊断,治疗和预后具有重要临床意义。 Objective To analyze the clinical significance of serum min/creatinine ratio in the early damage of diabetic nephropathy. Methods cystatin C and urinary albu- From April 2015 to March 2014, 95 cases of diabetes were selected. According to the urinary albumin excretion rate, the patients were divided into urine protein negative group (group A, 44 cases), and urine protein positive group ( group B, 51 cases). Seventy-two cases of health examination in our hospital were selected as the control group. Benckman automatic biochemical analyzer was used to determine serum CysC, urea and creatinine, urinary albumin/creatinine ratio content. Results The serum concentrations of CysC and the content of urinary albumin/creatinine ratio in early renal damage of type 2 DM was higher than that of the normal control group(P 〈0. 01 ). The serum CysC content and urinary albumin/creatinine ratio in group B were higher than those in group A (P 〈 0. 05). Conclusions CysC and U-mAlb/Cr can be used as sensitive and reliable indicators of early renal athy. Combined detection has important clinical significance betic nephropathy, treatment and prognosis. damage in the diagnosis in the diagnosis of early of diabetic nephroprenal damage in diabetic nephropathy, treatment and prognosis.
