甲状腺自身抗体与复发性自然流产临床相关因素的分析研究

Clinical correlative factors analysis between thyroid autoantibodies and recurrent spontaneous abortion

目的:研究甲状腺自身抗体(TA)与复发性自然流产(RSA)之间关系,为临床治疗提供依据。方法:选取2014年2月至2015年3月医院诊治的100例有RSA史的非孕期患者资料进行分析,将其设为实验组,选取同期入院100例有正常分娩史的非孕期健康妇女设为对照,两组入院后均采用化学发光法检测血清中甲状腺球蛋白抗体(Tg Ab)和甲状腺过氧化物酶抗体(TPOAb),分析甲状腺自身抗体(TA)与复发性自然流产(RSA)之间关系。结果:实验组TA总阳性率、Tg Ab阳性率、TPOAb阳性率,显著高于对照组(P<0.05);Logistic回归分析结果显示:实验组TPOAb阳性比例为对照组的3.439倍。实验组和对照组lg(Tg Ab)均数分别为(1.346±0.440)、(1.291±0.257)(t=1.077,P=0.283);实验组和对照组lg(TPOAb)均数分别为(0.928±0.649)、(0.731±0.274)(t=2.817,P=0.006)。结论:甲状腺自身抗体是复发性自然流产的独立危险因素,应该采取有效降低复发性自然流产发生率。

Objectives： To study the relationship between thyroid autoantibodies（ TA） and recurrent spontaneous abortion（ RSA）,so as to provide the basis for clinical treatment. Methods： The clinical data of 100 non-pregnant patients with a history of RSA from February 2014 to March 2015 was analyzed as the experimental group.Another 100 non- pregnant healthy women with a normal birth history in the same period were selected as the control group. Both groups were examined using chemiluminescence,to detect the serum thyroglobulin antibody（ Tg Ab） and thyroid peroxidase antibody（ TPOAb）,and analyze the relationship between TA and RSA. Results：The total TA positive rate,Tg Ab- positive rate and TPOAb positive rate of the experimental group was significantly higher than the control group（ P〈0. 05）; logistic regression analysis showed that the TPOAb positive ratio of the experimental group was 3. 439 times of the control group. The lg（ Tg Ab） mean value in experimental and control groups was（ 1. 346 ± 0. 440） and（ 1. 291 ± 0. 257） respectively（ t = 1. 077,P = 0. 283）; and the lg（ TPOAb）mean was respectively（ 0. 928 ± 0. 649） and（ 0. 731 ± 0. 274）（ t = 2. 817,P = 0. 006）. Conclusion： TA is an independent risk factor for RSA,which should be taken to reduce the incidence of RSA.
