程序冷冻法冷冻精子进行IVF-ET的临床结局分析 被引量:1

摘要 目的:探讨程序冷冻法冷冻精子在体外受精-胚胎移植(IVF-ET)中的临床结局及安全性。方法:回顾分析在本中心进行IVF-ET治疗的不孕夫妇的372个周期,分为4个组:用程序冷冻法冷冻精子行常规IVF共76个周期(A组),程序冷冻法冷冻精子行ICSI共92个周期(B组),同期新鲜精液行常规IVF周期97例(C组),新鲜精液行ICSI周期107例(D组),分别比较A组与C组、B组与D组的正常受精率、异常受精率、卵裂率、优质胚胎率、妊娠率、种植率、流产率、多胎率和畸形率。结果:与新鲜精液IVFET组相比较,程序冷冻法冷冻精子常规IVF组及ICSI组的正常受精率、异常受精率、卵裂率、妊娠率、种植率、流产率、多胎率和畸形率等方面差异无统计学意义(P>0.05);但冷冻精子行ICSI组与新鲜精子行ICSI比较,优质胚胎率低且有统计学意义(P<0.05)。结论:用程序冷冻法冷冻精子助孕可以获得与新鲜精子相似的结局,是一种较为安全可靠的精液保存方法,可用于IVF-ET术前精液冷冻保存。 Objectives: To explore the clinical application effect of programmable freezer method on cryopreservation of human sperm for in vitro fertilization and embryo transfer. Methods: A retrospective cohort study was conducted of the 372 cycles of IVF- ET of patients treated in our center. Patients were decided into four groups.Patients in group A underwent 76 cycles of IVF using programmable freezer method on cryopreservation of human sperm. Group B underwent 92 cycles of ICSI using programmable freezer method on cryopreservation of human sperm. Group C underwent 107 cycles of IVF using fresh sperm. Group D underwent cycles of ICSI using fresh sperm. The normal fertilization rate,appearance rate of ≥3PN,clinical pregnancy rate,implantation rate,abortion rate,multiplets rate and deformity rate was respectively compared between group A and group C,group B and group D. Results: No difference was observed among the four groups except for clinical pregnancy rate. Clinical pregnancy rate of group B was significantly lower than that of group D( P〈0. 05). Conclusion: Using the programmable freezer method on cryopreservation of human sperm can gain fairly good clinical effect which can be used in IVF-ET preoperative semen cryopreservation.
