慢性肾脏病3~5期未透析患者贫血情况调查及影响因素分析 被引量:5

摘要 目的:了解慢性肾脏病(CKD)3~5期未透析患者的贫血情况及其影响因素。方法对198例初治的住院CKD 3~5期未透析患者的临床资料进行回顾性分析,常规检测其血红蛋白(Hb)、红细胞压积(HCT)、尿素氮(BUN)、肌酐(Scr)、白蛋白(Alb)、全段甲状旁腺激素( iPTH)、血清铁蛋白( SF)、血清铁( SI)、总铁结合力( TIBC)、C 反应蛋白(CRP)、血沉(ESR)等指标。根据 SI 和 TIBC 数值计算转铁蛋白饱和度(TSAT),采用 Cockcroft - Gault(C - G)公式估算GFR(eGFR)。调查 CKD 患者贫血患病率,并对肾性贫血发生的危险因素进行分析。结果198例 CKD 患者中贫血149例(75.25%)。CKD 3~5期患者贫血患病率依次为29.41%(15/51)、77.08%(37/48)和97.98%(97/99)。单因素分析表明,BUN、Scr、iPTH、SI、TIBC、UIBC、CRP、ESR 及 eGFR 均与肾性贫血的发生相关(P ﹤0.05)。Logistic 回归分析表明肾功能减退、铁缺乏是肾性贫血发生的危险因素。结论 CKD 3~5期未透析患者贫血患病率高,肾功能减退、铁缺乏是肾性贫血发生的危险因素。
