五苓散对急性冠脉综合征患者经皮冠状动脉介入术后发生对比剂肾病的影响 被引量:7

摘要 目的观察中药五苓散对急性冠脉综合征患者经皮冠状动脉介入术(PCI)后发生对比剂肾病(CIN)的影响。方法将拟行PCI的急性冠脉综合征患者80例按随机数字表法随机分为中药组和对照组,各40例。两组均给予常规水化治疗,中药组给予五苓散方口服,择期手术患者术前24 h开始,至术后72 h;急诊PCI患者于术后2 h开始,至术后7 h。记录PCI术前和术后48~72 h血清肌酐(Scr)、尿中性粒细胞明胶酶相关脂质运载蛋白(NGAL)水平,估算肾小球滤过率(eGFR),观察CIN的发病率。结果中药组CIN的发生率为2.50%,明显低于对照组的5.00%(P〈0.05);PCI术前、术后肾功能指标比较,中药组在Scr、NGAL和eGFR 3项指标上均明显优于对照组(P〈0.05)。结论中药五苓散治疗对急性冠脉综合征患者经皮冠状动脉介入术后CIN的发生具有良好的预防作用。
