右美托咪定对患者抗伤害保护作用的影响

摘要 目的观察术前输注右美托咪定后患者受伤害刺激时灌注指数(PI)及脑电指数(NI)变化来评估右美托咪定的抗伤害保护作用。方法 40例行择期全身麻醉患者随机分配为D组和C组。插管前D组给予右美托咪定1μg/kg在15 min内泵注,C组给予泵注相同容量的生理盐水。记录基础值(t_0)、泵注右美托咪定后3 min(t_1)、6 mim(t_2)、9 min(t_3)、12 min(t_4)及15 min(t_5)、诱导后(t_a)、插管后20 s(t_(a1))、切皮时(t_b)、气腹(t_c)时各时间段的心率(HR)、血压(MAP)及NI值及PI值的变化。结果 D组t_3、t_4、t_5 NI值明显低于t_0组及同期C组(P均<0.05);D组与C组t_a、t_a1、t_b、tcNI值均明显较t_0明显下降,但2组比较差异无统计学意义(P均>0.05)。C组t_a、t_a1及t_b、t_c时PI明显低于t_0及同期D组(P均<0.05),D组较t_0显著升高(P均<0.05)。D组t_2、t_3、t_4、t_5及t_b、t_c HR较t_0及同期C组明显降低(P均<0.05)。C组t_3、t_5 HR较t_0明显升高(P均<0.05)。结论右美托咪定可以明显降低患者NI,抑制患者气管插管、切皮及气腹的刺激。NI指数可以反映右美托咪定的镇静效果,但却不能反映其对伤害刺激的保护效果。
