摘要 目的探讨3.0T MR多序列成像在急性脑梗死患者微出血灶检出中的价值。方法 选取2014年1月~2015年1月于中国石油中心医院行3.0T MR多序列成像扫描的急性脑梗死合并脑微出血(CMBs)患者53例,对CMBs在各个序列中的影像表现及位置进行分析研究,并对比各个序列对CMBs的检出率。结果 53例患者共检出201个CMBs,其中T1WI序列检出CMBs病灶20个,检出率为9.95%;T2WI序列检出CMBs病灶36个,检出率为17.91%;T2WI FLAIR序列检出CMBs病灶让48个,检出率为23.88%;DWI检出CMBs病灶139个,检出率为69.15%;SWI序列检出CMBs病灶201个,检出率为100.00%。将SWI序列对CMBs的检出率与各个常规MR扫描序列(T1WI、T2WI、T2WI FLAIR、DWI)对CMBs的检出率做两两比较,差异均有高度统计学意义(P〈0.01)。结论 3.0T MR多序列成像的应用明显提高了对CMBs病灶的检出率及准确性,对急性脑梗死患者的治疗及预测预后具有一定的临床价值。 Objective To explore the clinic value of 3.0T MR SWI in diagnosis of cerebral microbleeds (CMBs) in patients with acute cerebral infraction. Methods 53 cases of patients in the Central Hospital of China National Petroleum Corporation from January 2014 to January 2015 were selected, who had been diagnosed of acute cerebral infraction combined with CMBs by 3.0T MR SWI. The imaging founding and position of CMBs in each sequence were analyzed. The detection rates among each sequence were compared. Results Among 53 cases of patients, 201 CMBs lesions had been detected, among which 20 (9.95%) CMBs lesions had been detected by TIWI; 36 (17.91%) had been detected by T2WI; 48 (23.88%) had been detected by T2WI FLAIR sequence; 139 (69.15%) had been detected by DWI; 201 (100.00%) had been detected by SWI. The CMBs detection rates between MR scanning sequences (T1WI, T2WI, T2WI FLAIR, DWI) and SWI were compared, and the differences were statistically significant (P 〈 0.01). Conclusion Appli- cation of 3.0T MR SWI can significantly improve the detection rate and accuracy of CMBs, which has certain clinical value in treatment and predicting prognosis for patients with acute cerebral infraction.
