综合护理在肛裂术后合并便秘患者中的应用

Application of comprehensive nursing in postoperative anal fissure patients with constipation

目的 探讨综合护理对肛裂术后合并便秘患者的临床疗效。方法 选择新疆医科大学第一附属医院2014年7月～2015年7月收治的130例患者随机分为观察组（n=65）和对照组（n=65）。所有患者均行肛裂术,对照组给予常规护理措施,观察组给予综合护理措施。观察比较两组患者术后便秘情况及护理满意度,记录术后下床时间及住院时间。结果 观察组患者3 d内首次排便34例,占52.31%,明显高于对照组的24.62%;同时,观察组患者排便费力、粪便干硬及肛门堵塞感等症状的出现例数明显低于对照组,差异均有统计学意义（P〈0.05）。观察组患者下床时间和住院时间分别为（7.86±3.11）h和（4.54±1.65）d,明显低于对照组的（15.83±3.28）h和（8.19±2.33）d,差异均有统计学意义（P〈0.05）。观察组满意度为95.38%,明显高于对照组的73.85%,差异有统计学意义（Z=-3.469,P〈0.05）。结论 肛裂术后合并便秘患者的综合护理疗效显著,患者便秘情况得到有效改善,下床时间及住院时间明显缩短,护理满意度高,值得推广应用。

Objective To investigate the clinical efficacy of comprehensive nursing in postoperative anal fissure patients complicated with constipation. Methods 130 cases in the First Affiliated Hospital of Xinjiang Medical University from July 2014 to July 2015 were selected and randomly divided into observation group （n=65） and control group （n= 65）. All patients were underwent the anal fissure surgery, and then the control group was managed with conventional nursing, and the observation group was managed with comprehensive nursing. The postoperative constipation and nurs- ing satisfaction were compared, meanwhile, the postoperative ambulation time and hospitalization time were recorded. Results In observation group, 34 cases, accounting for 52.31%, defecated for the first time within 3 days, which was significantly higher than that of 24.62% in control group. Meanwhile, the number of defecation exertion, dry and hard stool and a sense of anal blockage in observation group was significantly lower than that of control group, with statistical significance （P 〈 0.05）. The ambulation time and hospitalization time was （7.86±3.11） h and （4.54±1.65） d respectively in observation group, which were significantly lower than those of （15.83±3.28） h and （8.19±2.33） d in control group （P 〈 0.05）. The nursing satisfaction of 95.38% in observation group was significantly higher than that of 73.85% in control group, with statistical differences （Z=-3.469, P 〈 0.05）. Conclusion It has a obvious clinical effect for comprehensive nursing in postoperative anal fissure patients with constipation. The constipation can be effectively improved, the ambulation time and hospitalization time are significantly shorter, and the nursing satisfaction is high. It is worthy of popularization and application.
