乳晕瓣与乳腺组织瓣联合矫正器外悬吊治疗乳头内陷的疗效观察

The Effectiveness of Areola and Breast Tissue Flap Valve Joint Braces Hanging Outside for Correction of Inverted Nipple

目的探讨采用乳晕瓣与乳腺组织瓣联合矫正器外悬吊治疗乳头内陷的疗效。方法按设计画线切开乳晕皮肤,在皮下进行锐性分离,形成两个乳晕瓣,在乳头基底钝性分离挛缩的肌肉结缔组织,切断短缩的纤维束,松解对乳头的牵拉,在乳头及乳晕内下方设计乳腺组织旋转瓣,将其充分地填充至复位的乳头下方空隙,缝合切口,用自制矫正器再适度牵引乳头2~3周。结果 2014年3月~2015年3月,我院采用乳晕瓣与乳腺组织瓣联合矫正器外悬吊治疗先天性乳头内陷患者15例（27侧）,随访2~6个月,平均4个月,全部患者术后伤口均Ⅰ期愈合,无乳头、乳晕血运障碍,无复发,切口瘢痕不明显。乳晕清晰,乳头外形基本良好,15例患者乳头凸出高度平均为8.0mm,患者表示满意,达到了美容与矫正为一体的功效,符合现代美容整形的理念。结论乳晕瓣与乳腺组织瓣联合矫正器外悬吊是治疗先天性乳头内陷较为理想的方法,手术创伤小,矫正幅度可操控性强,可达到修饰性矫正修复的目的。

Objective To explore the effectiveness of areola and breast tissue flap valves co outer suspension for correcting inverted nipples. Methods According to the design drawing line,areola skin was cut open,sharp separation was conducted in the subcutaneous and two areola flaps were formed. In the basilar papilla bluntly separate contracture of muscle connective tissue and cut the shortening of the fiber bundle,loose the nipple traction,rotation flap was designed in the nipple and areola below the breast tissue,the fully filled to reset below the nipples papilla void,small incision,with self-made appliance and moderate pulling the nipple for 2 to 3 weeks. Results From March 2014 and March2015,15 cases of patients（ 27 sides） were treated with areola and breast tissue flap valves co outer suspension to correct congenital inverted nipple,they were followed up for 2 to 6 months,with an average of 4 months. All patients were healed at the first stage,without the nipple and areola blood circulation disorder,no recurrence,no obvious scar. The areola was clear,the nipple shape was basically good and nipple protrusion average height of 15 cases of patients was 8. 0mm,all patients were satisfied,for beauty and correction as one of the effects,in line with the concept of modern cosmetic plastic surgery. Conclusion The areola and breast tissue flap combined aligner treatment outside suspension is an ideal way in treatment of congenital nipple retraction,with small surgical trauma,good controllability,and maneuverability in correction amplitude,and can be manipulated to achieve the purpose of modifying the correction fixes.
