16排螺旋CT多期增强扫描对胃癌的诊断价值

目的:对经胃镜及病理诊断确诊为胃癌的患者应用16排螺旋CT进行多期增强扫描,以探讨其临床诊断价值。材料与方法:选择我院60例胃癌患者术前应用16排螺旋CT进行多期增强扫描检查,进行各期图像重建以分析影像学特点。结果:60例胃癌患者中42例以门静脉的强化强度为主,12例动脉期的强化程度略高于门静脉期,6例以平衡期强化程度最为明显,平均强化程度均以门静脉期最高,与动脉期对比具有显著性差异(p<0.05);多期增强扫描后对胃癌分型的总检出率为85.5%;对胃癌淋巴结转移的诊断符合率为87.3%,敏感度为86.6%,特异度为87.2%;对胃癌TNM分期的准确率为80.0%。结论:在临床对胃癌术前患者应用16排螺旋CT进行多期增强扫描可较为准确的判断大体分型,进一步提高对胃癌淋巴结转移诊断的准确性,并且对胃癌术前TNM分期的诊断准确率较高,因此应用价值较高。

Purpose： gastric cancer diagnosed by endoscopy 16 patients with multi-slice spiral CT scan of enhance- ments to investigate the clinical diagnostic value. Materials and Methods： 60 eases of gastric cancer in our hospital patients before use 16-slice spiral CT multi-phase enhanced scan, image reconstruction performed to analyze each of the imaging fea- tures. Results ： 60 cases of gastric cancer in 42 cases to enhance the strength of the main portal vein, and arterial phase con- trast significant difference （ p 〈 0.05 ） ; after multi-phase enhanced scanning type of gastric cancer detection rate of 85.5% of the total; for lymph node metastasis diagnosis was 87. 3% , sensitivity 86. 6% , specificity was 87.2% ; accurate TNM staging of gastric cancer was 80. 0%. Conclusion ： The clinical TNM staging of gastric surgery diagnostic accuracy of patients with gastric surgery performed 16-slice spiral CT multi-phase enhanced scan can more accurately determine the general classifica- tion, to further improve the accuracy of diagnosis of lymph node metastasis, and higher, so a higher value.
