肾上腺皮质腺癌螺旋CT多期增强扫描的影像特点及诊断价值

Imaging Features and Diagnostic Value of Spiral CT Enhanced Scanning in the Adrenal Cortex

目的:探讨肾上腺皮质腺癌螺旋CT多期增强扫描的影像特点及诊断价值。材料与方法:回顾性分析23例经手术病理证实的肾上腺皮质腺癌的CT资料。结果:23例中,功能性皮质腺癌14例,无功能皮质腺癌9例。23例肾上腺皮质腺癌,其中15例位于左侧肾上腺,8例位于右侧肾上腺,均为单发,肿瘤形态呈类圆形,略有分叶。肿瘤直径在3.2cm-16.3cm之间,23例中21例肿瘤密度不均,可见低密度坏死区域,5例肿瘤内见钙化。增强检查肿瘤实质成分明显或中度强化,6例肿瘤内可见肿瘤血管影。23例中17例与邻近正常组织界限不清,5例存在远处转移。结论:螺旋CT多期增强扫描能获得肾上腺皮质腺癌的强化特征,对肾上腺皮质腺癌的诊断有着重要的价值。

Purpose： to investigate the imaging features and diagnostic value of spiral CT in the diagnosis of adrenal gland carcinoma. Materials and Methods： CT data of 23 cases of adrenal cortical carcinoma confirmed by operation and pa- thology were retrospectively analyzed. Results： in 23 cases, 14 cases of functional cortical adenocarcinoma, 9 cases of non functional cortical adenocarcinoma. 23 cases of adrenal cortical adenocarcinoma, 15 cases located in the left adrenal gland, 8 cases located in the fight adrenal gland, were single, tumor morphology was round, slightly different. Tumor diameter was be- tween 3.2cm- 16. 3cm, 23 cases in 21 cases of tumor density were not all, visible low density necrosis region, 5 cases of tumor calcification. Enhanced examination of tumor parenchyma in moderate or marked enhancement, 6 cases of tumor angio- genesis. In 23 cases, 17 cases were not clear with the adjacent normal tissues, and 5 cases had distant metastasis. Conclu- sion ： the enhanced CT scan can obtain the enhanced features of adrenal cortical adenocarcinoma, and it has important value in the diagnosis of adrenal cortical adenocarcinoma.
