脊髓磁共振水成像在自发性颅内低压脊髓脑脊液漏点治疗术后的影像表现 被引量:1

摘要 目的探讨脊髓磁共振水成像(MR myelography,MRM)在自发性颅内低压(Spontaneous intracranial hypotension,SIH)脊髓脑脊液漏靶向硬膜外血贴(EBP)手术前后的影像表现及诊断价值。方法回顾性分析80例接受靶向EBP治疗并获得临床痊愈SIH患者的影像资料,对其中62例同时具备靶向EBP手术前后的MRM检查资料进行对比分析评估。结果 62例SIH患者中,对比靶向EBP手术前后MRM影像表现变化特征为:(1)直接征象:脊髓脑脊液漏口消失伴脑脊液异常积聚减少消失:术前明确检出漏口患者47例,脑脊液椎体旁异常积聚检出62例,术后47例患者漏口均未见显示,术后脑脊液积聚量减少或异常积聚消失60例,无明显变化2例;(2)间接征象:颈段硬膜囊内静脉迂曲扩张减轻或消失:靶向EBP术前MRM检出椎管硬膜囊内静脉迂曲扩张15例,术后硬膜囊内静脉迂曲扩张程度明显减轻或消失13例,2例无明显变化;神经根袖边缘形态变光整:靶向EBP术前MRM检出神经根袖边缘形态模糊38例,术后神经根袖边缘光整28例,10例无明显变化。结论靶向EBP治疗术后获得临床痊愈的SIH患者MRM表现较术前具有特征性变化,MRM可作为临床疗效评估重要手段。
