社区家庭医生服务模式对糖尿病、高血压患者的影响

Influence of community family doctor service model for diabetes,high blood pressure patients

目的探讨社区家庭医生服务模式对糖尿病、高血压患者的影响。方法选取本社区2013年1月-2015年12月的257例高血压或糖尿病患者作为研究对象,随机分为对照组（125例）和观察组（132例）。两组均给予自行药物治疗,对照组仅在家休养,观察组给予社区家庭医生服务模式干预,比较两组的干预效果。结果观察组干预后的饮食控制、相关知识掌握情况、规律用药情况以及参加运动率显著高于对照组,差异有统计学意义（P〈0.01）。观察组的糖尿病有效控制率为97.22%,显著高于对照组的76.67%,差异有统计学意义（P〈0.05）。观察组的高血压有效控制率为94.79%,显著高于对照组的81.05%,差异有统计学意义（P〈0.01）。观察组治疗后的空腹血糖、餐后2 h血糖、血压水平显著低于对照组,差异有统计学意义（P〈0.01）。结论社区家庭医生服务模式能够更好地服务于糖尿病、高血压患者,改善其不良生活习惯,提高治疗效果和生活质量,值得临床推广应用。

Objective To explore the influence of community family doctor service model for diabetes,high blood pressure patients. Methods 257 cases suffering from high blood pressure or diabetes patients in our community from January 2013 to December 2015 were selected and randomly divided into the control group（125 cases） and the observation group（132 cases）.Both groups were given their own drug treatment,the control group was only rested at home,the observation group was given community family doctor service model to intervene.The intervention effet in the two groups was compared. Results The rate of diet control,the related knowledge mastery,the regular use of drug and the movement after intervention in the observation group was higher than that in the control group respectively,with significant difference（P〈0.01）.The effective control rate of diabetes in the observation group was 97.22%,which was higher than 76.67%in the control group,with significant difference（P〈0.05）.The effective control rate of blood pressure in the observation group was 94.79%,which was higher than 81.05% in the control group,with significant difference（P〈0.01）.The level of fasting blood glucose,postprandial 2 hours blood glucose,blood pressure after treatment in the observation group was lower than that in the control group,with significant difference（P〈0.01）. Conclusion Community family doctor service model can better serve diabetes,hypertension patients,improve the bad habits of patients,treatment effect and quality of life,it is worthy of clinical promotion and application.
