摘要
目的探讨Gene Xpert MTB/RIF技术在肺结核诊断及利福平耐药检测中的意义。方法选取我院120例疑似结核病患者,根据临床诊断结果将其均分为结核组和非结核组,两组患者均进行Gene Xpert MTB/RIF实验、T-spot实验、液体培养以及药敏试验,比较各项检测的敏感性和特异性。结果 Gene Xpert MTB/RIF法、T-spot、痰涂片、液体培养检测结核分枝杆菌敏感性分别为71.67%、66.67%、51.67%、53.33%,特异性分别为96.67%、95.00%、96.67%和100.00%,其中Gene Xpert MTB/RIF法检测的敏感性明显高于痰涂片、液体培养法(P均<0.05)。与传统药敏试验比较,Gene Xpert MTB/RIF检测利福平耐药结核病的敏感性和特异性分别为83.33%、90.00%。结论 Gene Xpert MTB/RIF技术在肺结核诊断及利福平耐药检测中具有较好的敏感性和特异性,值得推广。
Objective To investigate the significance of GeneXpert MTB/RIF technique in diagnosis of tuberculosis (TB) and detection ofrifampin resistance. Methods 120 TB suspects in our hospital were selected and divided into TB group (60 cases) and non-TB group (60 cases) according to clinical diagnosis results. All patients were performed GeneXpert MTB/RIF test, T-spot test, liquid culture and drug sensitive test (DST), and the sensitivity and specificity of above tests were compared. Results The sensitivity of GeneXpert MTB/RIF, T-spot, sputum smear, liquid culture for detection of Mycobacterium tuberculosis were 71.67%, 66.67%, 51.67%, 53.33% respectively, and the specificity were 96.67%, 95.00%, 96.67% and 100.00% respectively; the sensitivity of GeneXpert MTB/RIF was higher than that of sputum smear and liquid culture (both P〈0.05). Compared with traditional DST, the sensitivity and specificity of GeneXpert MTB/RIF were 83.33% and 90.00% respectively. Conclusions Genexpert MTB/RIF has better sensitivity and specificity in TB diagnosis and rifampin resistance detection, which is worthy of promotion.
