气管支气管树腺样囊腺癌的CT诊断

Adenoid cystic carcinoma of the tracheobronchial tree: CT features

目的研究CT对气管支气管树腺样囊腺癌的诊断特征和临床价值。方法对2例气管支气管树腺样囊性癌的患者进行了临床和CT征象观察,对该病CT特征结合临床表现、病理组织改变进行总结与讨论。结果 2例患者以咳嗽、咯血及呼吸困难为主要临床表现,CT和后处理技术可准确显示癌肿环绕气管、主支气管和段支气管管壁纵向生长、管壁环形增厚、狭窄、闭塞和腔外重要结构侵犯。结论沿气道长轴生长伴管壁增厚是腺样囊腺癌特征性的CT表现,CT和后处理重建可清楚显示病变的生长类型和范围,有助于临床诊断和治疗。

Objective To study the characteristics and the diagnostic value of CT in the tracheobronchial tree of adenoid cystic carcinoma（ ACC）. Methods Two patients with ACC were observed in aspects of CT imaging,and the clinical manifestations and pathological changes were summarized and discussed as well. Results The main symptoms of the two patients with ACC were cough,hemoptysis and progressive inspiratory dyspnea. CT scans combined with multiplanar,minimum-intensity-projection,and3 D reconstructions accurately showed the lesions involved the tracheal and the main bronchial walls with irregular ring-type thickness,narrowness,occlusion and extra-luminal important structure involvement. Conclusion The infiltrative wall thickening along the large airways were CT characteristics of the ACC. CT with combined with use of postprocessing can display clearly the growth pattern and invasive extension of the lesions and were useful to clinical diagnosis and management.
