摘要 目的探讨高分辨磁共振腹部弥散加权像(DWI)测量表观扩散系数(ADC)评价男、女性腰椎骨质疏松的价值。方法 63例受试者用双能X线吸收仪(DXA)测量腰1~4(L1-4)椎体骨密度(BMD)和DWI测量ADC值;男、女性L1-4各椎体ADC值与BMD值、平均ADC值与总BMD、T值、年龄分别行线性相关分析;男、女性分别按T值分为骨质正常组、减少组和疏松组,方差分析比较ADC值的差异;ROC曲线分别评价ADC值对男、女性L1-4椎体骨质疏松的诊断价值。结果 L1-4平均ADC值男性(0.320±0.190)×10-3mm^2/s与BMD、T值、年龄无相关性(r=0.033、0.038、-0.106,P>0.05),女性(0.281±0.091)×10-3mm^2/s与BMD、T值正相关(r=0.370、0.346,P﹤0.05),与年龄负相关(r=-0.54,P﹤0.01);三组间差异比较男性无统计学意义(P>0.05),女性有统计学意义(P﹤0.05);ROC曲线分析女性组有诊断意义,且L3水平曲线下面积(AUC=0.929)最大,男性组无意义。结论 ADC值对女性腰椎骨质疏松具有一定的诊断价值。 Objective To explore the value of the apparent diffusion coefficient( ADC) in abdominal high-resolution diffusion-weighted MR Imaging in evaluating the lumbar vertebral osteoporosis of the male and female. Methods 63 subjects were measured the bone mineral density( BMD) by dual energy X-ray absorptiometry and ADC by abdominal MR Imaging at the site of lumbar spine( L1-4). Linear correlation was applied to analyze the relationship between the ADC and BMD values of every L1-4vertebral,the average of ADC values and total BMD,T score and age of Male and female,respectively. Males and females were divided into normal,osteopenia and osteoporosis groups based on the T score respectively,variance analysis was used for ADC comparison between groups. The receiver-operating characteristic( ROC) curve was used to evaluate the diagnostic value of osteoporosis at L1-4by ADC for male and female respectively. Results No significant correlation was present between the average of ADC 〔( 0. 320 ± 0. 190) × 10-3mm^2/ s〕at L1-4and BMD,T score,age( r = 0. 033,0. 038,0. 033,P〈0. 05) in male group.But there was a positive correlation between average of ADC 〔( 0. 281 ± 0. 091) × 10-3mm^2/ s〕at L1-4and BMD,T score( r =0. 370,0. 346,P ﹤ 0. 05) in female group,and a negative correlation with the age( r =-0. 54,P ﹤ 0. 01). There was no statistically significant difference among the three male groups( P〈0. 05),and a statistical significance difference among the three female groups( P ﹤ 0. 05). ROC curve analysis showed that female group had a good diagnostic value,and area under curve( AUC = 0. 929) of L3 was the largest,there was no value in male group. Conclusion To a certain extent,ADC has diagnostic value of lumbar vertebral osteoporosis for women.
