慢性乙型肝炎患者的个性、情绪、应对方式及家庭支持对生活质量的影响

Effect of personality,emotion,coping style and family support on quality of life of patients with chronic hepatitis B

目的探讨慢性乙型肝炎(CHB)患者的个性、情绪、应对方式及家庭支持对生活质量的影响。方法采用问卷调查、艾森克人格测验(EPQ)、抑郁自评量表(SDS)、焦虑自评量表(SAS)、特质应对方式问卷(TCSQ)、领悟社会支持量表(PSSS)和世界卫生组织生活质量测定简表(WHOQOLBREF)对68例CHB患者(CHB组)和74例正常人(对照组)进行了测评,分析其与生活质量的相关关系。结果 CHB组生活质量总分(66.76±15.68)分、生理领域(13.49±2.67)分和心理领域(14.51±2.13)分,均低于对照组的(79.21±8.23)分、(14.51±2.13)分和(15.39±2.03)分(P<0.05)。Pearson相关分析显示,生理、心理领域与PSSS的家庭外支持因子呈正相关,与SAS总分、SDS总分、EPQ中神经质维度和TCSQ的消极应对因子呈负相关(P<0.05或P<0.01))。多元线性逐步回归分析结果显示,食欲因子、SDS总分和家庭外支持因子可预测生活质量的生理领域,家庭外支持因子、神经质因子和食欲因子可预测生活质量的心理领域。结论 CHB患者的生活质量明显低于普通人,尤其在生理和心理领域,与抑郁焦虑情绪、神经质人格、消极应对方式、家庭外支持密切相关。

Objective To explore the impact of personality,emotion,coping style and family support on the quality of life in the patients with chronic hepatitis B（CHB）.Methods The investigations were performed by Eysenck personality questionnaire（EPQ）,self-rating depression scale（SDS）,self-rating anxiety scale（SAS）,trait coping style questionnaire（TCSQ）,perceived social support scale（PSSS）,and WHO Quality of Life Scale Brief Version（WHOQOL-BREF）in 68 cases of CHB patients（group CHB）and 74 healthy people（group C）.Results The scores of quality of life,physical domain,and psychological domain were（66.76±15.68）points,（13.49±2.67）points and（14.51±2.13）points in group CHB,which were lower than（79.21±8.23）points,（14.51±2.13）points and（15.39±2.03）points in group C（P〈0.05 or P〈0.01）.Pearson correlation analysis showed that the physiological and psychological fields were positively correlated with PSSS support factor outside family.Stepwise multiple linear regression analysis showed that appetite factor,score of SDS and family support factor could predict the quality of life physiological domain,and the factors of family support,nervosity and appetite could predict the quality of life psychological domain.Conclusion The life quality of CHB patients is significantly lower than that of healthy people,which especially in the field of physiology and psychology is closely associated with the anxiety and depression,neurotic personality,negative coping style and family support.
