乳腺血氧功能影像检查仪联合钼靶X线检查在乳腺癌筛查中的作用

The application of breast blood oxygen functional imaging combined with mammography examination in breast cancer screening

目的探讨乳腺血氧功能影像检查仪联合钼靶X线检查在40岁及以上体检妇女乳腺癌筛查中的作用。方法10 076名妇女进行乳腺癌筛查,均进行乳腺血氧功能影像检查仪及钼靶X线检查,将乳腺影像报告和数据系统(BI-RADS)Ⅲ级以上妇女进行的活检病理诊断结果作为"金标准",对比不同检查方法的灵敏度和特异度。结果 10 076名体检妇女中经乳腺血氧功能影像检查仪、钼靶X线诊断为BI-RADSⅢ级以上235例,其中早期乳腺癌13例,良性病变222例;乳腺血氧功能影像检查仪灵敏度为69.23%,特异度为85.59%;钼靶X线灵敏度为76.92%,特异度为82.88%;联合检查灵敏度为100.00%,特异度为82.88%;乳腺血氧功能影像检查仪联合钼靶X线的正确指数0.829高于单项检测的正确指数0.548和0.598(U=2.117、2.501,P<0.05)。结论乳腺血氧功能影像检查仪联合钼靶X线检查能有效提高40岁及以上体检妇女乳腺癌筛查的准确性。

Objective To evaluate the effect of breast blood oxygen functional imaging combined with mammography on breast cancer screening in women aged over 40 years. Methods A total of 10 076 cases of women in our hospital underwent breast blood oxygen functional imaging and mammography X- ray examination. The breast imaging report anddata system （ BI - RADS） Ⅲ level of women performed on the biopsy^ pathology were used as of diferent inspection methods were compared and analyzed. Results Among 10 076 caseBI - RADS,which was diagnosed by blood oxygen functional imaging system and a molybdenum taIII. Thirteen cases with early breast cancer were diagnosed by needle biopsy. And 222 cases were diagnosed benignlesions. The sensitivity of the blood oxygen functional imaging system was 69. 23% （9/13） and 85. 58% （ （90/222 ）,respectively. The sensitivity of molybdenum target X - ray was 76.92% （ 10/13 ） , and the speci222）. The sensitivity of combined detection was 100.0% （13/13） and the specificity was 82. 88% （184/222） . Theaccuracy index of combined molybdenum target X - ray （ 0. 829） was significantly higher than （0. 548 and 0. 598 ） （U = 2. 117,2. 5 0 1 ,P 〈 0. 05 ） . Conclusion Breast blood oxygen functional imaging instrument combined with molybdenum target examination could effectively improve the accuracy of diagnosis in breast cancer screening among women over 40 years.
