摘要
目的:观察针药复合麻醉应用于乳房良性肿瘤手术中的镇痛效果和安全性。方法:回顾性分析2006年12月—2013年5月在本科手术治疗的乳房良性肿瘤患者,针药复合组732例,常规使用利多卡因(1%利多卡因和生理盐水按1:1稀释备用)联合针刺麻醉(疏密波2/15 Hz 30min电流量,患者能耐受为宜),取穴:双侧合谷、内关、梁丘。对照组652例,常规使用1%利多卡因局部浸润。结果:1针药复合组术前、术中SBP、DBP、HR差异显著(P<0.05),局麻组术前、术中SBP、DBP、HR无差异(P>0.05)。2两组术前SBP、DBP、HR比较无差异(P>0.05),术中SBP、DBP、HR比较差异显著(P<0.05)。3两组治疗后疼痛评分比较差异显著(P<0.05)。4两组利多卡因用药量差异显著(P<0.05)。结论:针药复合麻醉可以减少利多卡因的用药量,应用于乳房良性肿瘤术中安全、有效。
Objective: To observe the analgesic effect and security of combined acupuncture and drugs applied to the surgery of benign breast tumor. Methods: Among the patients with benign breast tumor admitted into our hospital between December,2006 and May,2013 were conducted retrospective analysis. For the 732 cases performed combined acupuncture and drugs treatment,lidocaine was used in a routine manner( 1% lidocaine : normal saline = 1: 1); the selected acupoints include Hegu( LI4),Neiguan( PC6) and Liangqiu( ST34) on both sides with dilatational wave of 2 /15 Hz for 30 min and amperage within the patients’ tolerance. For the local anesthesia group of 652 cases,1% lidocaine was applied in a routine manner for local infiltration. Results: 1.The differences in SBP,DBP and HR before and during the surgery in combined acupuncture and drugs group were significant( P 〈 0. 05). There was no significant difference in SBP,DBP and HR in local anesthesia group before and during the surgery( P 〉 0. 05). 2. Before the surgery,the differences in SBP,DBP and HR between the two groups were not significant( P 〉0. 05). During the surgery,the difference in SBP,DBP and HR between the two groups was significant( P 〈 0. 05). 3. The differenceof the pain scores between the two groups were significant( P 〈 0. 05). 4. The difference of the dosage of drugs between the two groups was significant( P 〈 0. 05). Conclusion: Combined acupuncture and drugs anesthesia can reduce the dosage of lidocaine and is safe and effective applied to the surgery of benign breast tumor.
