四维彩超在产科胎儿畸形筛查中的应用分析

Application value of four-dimensional color Doppler ultrasound in the screening of fetal malformation in the department of obstetrics

目的分析四维彩超在产科胎儿畸形筛查中的应用价值。方法选择2013年10月至2014年11月在洛阳市妇女儿童医疗保健中心接受胎检的1 350例孕妇作为研究对象,分别使用二维彩超和四维彩超检查,比较两种超声成像技术诊断的准确性,追踪随访胎儿分娩后的健康状况。结果分娩后确诊胎儿畸形46例,其中面部畸形13例,脏器畸形11例,躯干畸形9例,脊柱畸形5例,四肢畸形4例,头颅畸形2例,胎盘及脐带畸形2例。普通二维超声和四维彩超检查分别实际检出37例和44例,诊断准确性分别为80.4%、95.7%。四维彩超检查诊断准确性高于普通二维超声检查,差异有统计学意义(P<0.05)。结论四维彩超应用于产科胎儿畸形筛查中能够显著提高诊断准确性,值得在临床产前筛查中推广应用。

Objective To analyze the application value of four -dimensional color Doppler ultrasound in the screening of fe-tal malformation in the department of obstetrics.Methods 1 350 pregnant women accepted both two -dimensional and four -di-mensional color Doppler ultrasonography in Women and Children Health Care Center of Luoyang from October of 2013 to Novem-ber of 2014.The diagnostic accuracies were compared and the health status of the fetus after delivery was followed up.Results There were 46 cases of fetal malformation confirmed after delivery,including 13 cases of facial deformity,11 cases of viscera de-formity,9 cases of trunk deformity,5 cases of spine malformation,4 cases of limb deformity,2 cases of cloverleaf skull,2 cases of placenta and umbilical cord deformity.There were 37 and 44 cases of fetal malformation confirmed by two -dimensional and four -dimensional color Doppler ultrasonography,respectively.The diagnostic accuracies of two -dimensional and four -dimen-sional color Doppler ultrasonography were 80.4% and 95.7% respectively,and the difference was statistically significant （P 〈0.05）.Conclusion Four -dimensional color Doppler ultrasound has a high diagnostic accuracy in the screening of fetal malfor-mation in the department of obstetrics,and deserves the clinical expansion.
