Mulligan技术配合按摩手法治疗颈型颈椎病的临床研究

Clinical study of Mulligan technology and massage in the treatment of neck type of cervical spondylopathy

目的:观察Mulligan技术在颈型颈椎病中的应用效果。方法:随机将56名颈型颈椎病患者分为2组,观察组采用Mulligan技术治疗,对照组采用关节松动术治疗。治疗前后分别用VAS评估疼痛的程度和用关节活动度测量评估颈椎活动度的变化,治疗结束后统计2组治疗有效率和治疗所需的次数,治疗结束后半年随访患者颈椎病复发的情况。结果:治疗后,2组VAS评分和关节活动度均较治疗前明显改善(P<0.01),观察组VAS评分更低于对照组(P<0.05),观察组的后伸和左旋转这两个活动方向比对照组改善更明显(P<0.05,0.01)。治疗后,观察组的治疗次数较对照组明显减少(P<0.01);观察组的有效率明显高于对照组(P<0.01);观察组的复发率明显低于对照组(P<0.01)。结论:Mulligan技术治疗颈型颈椎病是一种简便易行、安全有效的治疗方法,值得临床推广。

Objective：To observe the effects of Mulligan technology application in the neck type of cervical spondylopathy.Methods：Fifty-six patients with neck type of cervical spondylopathy were randomly divided into two groups.Treatment group was treated by Mulligan technology,while the control group was treated by joint mobilization.Before and after the treatment,VAS was applied to both groups to evaluate the degree of pain and the range of motion.The effective rate and number of treatment were summarized in both groups after treatment.Besides,the patients who were diagnosed with neck type of cervical spondylopathy were followed up for 6months to observe the recurrence.Results：After treatment,VAS score and the range of motion in the two groups had substantial improvement as compared with before（P〈0.01）.VAS score in the treatment group was less than in the control group（P〈0.05）.The cervical vertebra extension and left rotating motion in the treatment group also showed much more improvement than the control group（P〈0.05,0.01）.After the treatment,the treatment times in the treatment group were significantly reduced as compared with the control group（P〈0.01）.The effective rate in the treatment group was significantly higher than in the control group（P〈0.01）.The recurrence rate was significantly lower in the treatment group than in the control group（P〈0.01）.Conclusion：Mulligan technology is a user-friendly,safe and effective approach in the treatment of neck type of cervical spondylopathy,which is worthy of clinical promotion.
