原发性皮肤弥漫大B细胞淋巴瘤(腿型)1例

A Case of Primary Cutaneous Diffuse Large B Cell Lymphoma,Leg Type

患者男，89岁。左小腿结节、肿块2个月。皮肤科情况：左小腿伸侧可见一鸡蛋大小肿块，中央破溃、结痂。周边可见多个红色结节，质硬，伴轻度压痛。皮损活检示：真皮弥漫性淋巴样肿瘤细胞浸润，不累及表皮，与表皮间可见无浸润带。免疫组织化学示：CD3（-，CD20（＋），CD10（-），CD21（-），BCL-2（＋），BCL-6（-），MUM-（＋），Ki-67（80％～90％＋），CD30（-），CD79a（＋），EBER（-，ALK（-）。诊断：原发性皮肤弥漫大B细胞淋巴瘤（腿型）。

An 89-year-old male presented with a mass and nodules on his left lower leg for two months. Dermatological examination showed an egg-sized mass with ulcer and crust on the extensor surface of his left lower leg. Several hard and painful red nodules were observed around the mass. Skin biopsy and histopathology showed grenz zone between epidermis and dermis, diffuse lymphoid tumor cells infiltration in the dermis. Immunohistoehemistry demonstrated positive for CD20, CD79a, BCL-2, MUM-1, Ki-67 （ 80% ~ 90% ） ,and negative for CD3, CD10, CD21, CD30, BCL-6, EBER, ALK. A diagnosis of primary cutaneous diffuse large B cell lymphoma,leg type（PCDLBCLLT） was made.
