颈部坏死性筋膜炎7例并文献复习

Diagnosis and treatment of cervical necrotizing fasciitis: a report of 7 cases and literature review

目的探讨颈部坏死性筋膜炎的临床特点及治疗经验。方法 7例颈部坏死性筋膜炎患者均经颈部CT及术中所见确诊,其中行气管切开并行颈部切开引流术(其中1例为外院施行)2例,另仅行颈部切开引流术5例,术后冲洗换药并进行抗感染治疗。结果治愈6例,随访半年无复发;死亡1例。结论颈部坏死性筋膜炎患者确诊后应尽早采取手术探查,行颈部脓肿切开引流治疗,可获得满意疗效,并有助于预防严重并发症。

Objective To explore the clinical characteristic,diagnosis and treatment of cervical necrotizing fasciitis.Methods All 7 cases of cervical necrotizing fasciitis w ere diagnosed by CT and confirmed in operation. Of them,2underw ent tracheotomy,neck incision and drainage operation,and the other 5 received neck incision and drainage operation. Postoperative flushing and anti-infection treatment w ere administered. Results Of all cases,1 died and the other6 w ere cured. During the follow-up of half a year,no relapse occurred. Conclusion For cervical necrotizing fasciitis cases,explorative operation should be timely applied. Incision and drainage of neck abscess can obtain a satisfactory effect and prevent serious complications.
