输卵管插管联合宫腹腔镜治疗输卵管性不孕症疗效观察

目的探讨输卵管插管联合宫腹腔镜治疗不同部位输卵管性阻塞不孕的效果。方法经输卵管碘油造影诊断为双侧输卵管阻塞不孕症患者100例，行宫一腹腔镜下输卵管插管治疗，统计术后1个月输卵管通畅情况及妊娠情况。结果（1）输卵管插管联合宫腹腔镜治疗对于输卵管近端阻塞、远端阻塞均有治疗效果。（2）输卵管插管联合宫腹腔镜治疗对于输卵管近端阻塞治疗效果好于远端阻塞。（3）输卵管性不孕症患者年龄越大，术后成功率越小。（4）输卵管性不孕症患者术后半年内成功妊娠率最高，妊娠率随时间延长而下降。结论宫腹腔镜联合输卵管插管治疗输卵管不孕症效果较理想，对于不同部位阻塞均有治疗效果，使患者受孕率提高，值得临床推广。

Objective To investigate the clinical effect and application value of tubal intubation combined with hysteroscopy in the treatment of tubal obstruction infertility in different locations. Methods We selected 100 cases diagnosed of bilateral fallopian tube obstruction infertility patients with the fallopian tube lipiodol angiography from 369 infertility patients of our hospital from January 2000 to December 2014. Then we recorded fallopian tube patency and pregnancy situation after a month of hysteroscopy and laparoscopy and tubal intubation treatment. Results （ 1 ） tubal intubation combined with hysteroscopy and laparoscopy treatment for tubal occlusion of the proximal and distal obstruction has therapeutic effects. （2） Tubal intubation combined with hysteroscopy and laparoscopic treatment for proximal tubal occlusion therapy effect was better than the distal obstruction （ 3 ） the older tubal infertility patient＇ s age, the smaller postoperative success rate （ 4 ） the success of pregnancy rate was highest within six months of treatment, and the pregnancy rate decreased with time. Conclusion Tubal intubation combined with hysteroscopy is effective to tubal infertility in different parts, so that it is worthy of clinical promotion.
