锁骨钩钢板在肩锁关节脱位合并喙突撕脱骨折治疗中的应用

Effect analysis of clavicular hook plate in patients with acromioclavicular joint dislocation combined with coracoid avulsion fracture

目的：观察锁骨钩钢板治疗肩锁关节脱位合并喙突撕脱骨折的临床效果。方法：收集2010年5月至2014年10月治疗的肩锁关节脱位合并喙突撕脱骨折患者8例,所有患者均为TosssyⅢ型肩锁关节脱位,其中1例患者合并第2掌骨骨折,1例合并腕掌关节脱位,所有患者均采用锁骨钩钢板切开复位固定术,间接复位喙突、不固定,并发症采用非手术治疗,受伤至手术时间平均为3.7 d（3~7 d）。结果：8例患者手术进展均比较顺利,平均手术时间为59.5 min（47~68 min）,8例患者均获得随访,平均随访9.5月（8~12月）,依据肩关节Constant-Murley评分,优7例,良1例,优良率100%。结论：锁骨钩钢板治疗肩锁关节脱位合并喙突撕脱骨折复位良好,疗效较好,有很好的临床推广价值。

Objective：To explore the clinical effect of clavicular hook plate in patients with aeromioelavieular joint dislocation combined with eoraeoid avulsion fracture. Method：8 eases of aeromioelavicular joint dislocation combined with coraeoid avulsion fracture were collected from May 2010 to October 2014. All was Tosssy Ⅲ acromioclavieular joint dislocation, treated with clavicular hook plate, l ease was accompanied with the second metacarpal fractures. 1 case was accompanied with wrist joint dislocation. Coronoid was not fixed with indirect reduction. Complications were treated conservatively. The average time from injury to operation was 3.7 days （3 - 7 days）. Results：Surgical progress of all 8 patients was relatively smooth. The average operation time was 59.5 min （47 min - 68 min）. And all the patients were follow - up, and the average follow - up time was 9.5 months （8 - 12 months）. Aceroding to Constant -Murley score of shoulder joint, 7 eases recovered excellently, and 1 ease recovered well. The excellent and good rate of the treatment was 100%. Conclusion：The effect of clavicular hook plate in the patients of aeromioclavieular joint dislocation combined coracoid avulsion fracture is firm. Patients have adequate reduction, rigid fixation, little adverse reaction and it deserves clinical application and popularization.
