多种肿瘤标志物检测在肺癌辅助诊断中的价值

Value of multiple tumor markers in diagnosis of lung cancer

目的探讨血清组织多肽特异性抗原(TPS)、神经元特异性烯醇化酶(NSE)、癌胚抗原(CEA)、糖类抗原19-9(CA19-9)、细胞角蛋白片段抗原21-1(CYFRA21-1)检测在肺癌辅助诊断中的价值。方法选择100例肺癌患者(肺癌组)和100例健康体检者(对照组),采用电化学发光免疫法测定血清TPS、NSE、CEA、CA19-9及CYFRA21-1含量,分析TPS、NSE、CEA、CA19-9、CYFRA21-1标志物对肺癌诊断的灵敏性、特异性、准确性及诊断价值。结果肺癌组血清TPS、NSE、CEA、CA19-9及CYFRA21-1[分别为(166.23±81.62)U/L、(11.91±9.32)ng/ml、(10.31±6.36)ng/L、(16.19±6.71)U/L、(2.61±1.21)ng/L]含量均明显高于对照组[分别为(51.16±7.12)U/L、(7.31±6.22)ng/ml、(1.66±1.01)ng/L、(2.28±1.21)U/L、(0.36±0.26)ng/L](均P<0.01)。在肺癌的临床辅助诊断中,TPS的诊断灵敏性(80.0%)最高,NSE的特异性(96.0%)及准确性(73.0%)最高。在各项标志物中,TPS、NSE、CEA及CYFRA21-1对肺癌均有诊断价值,以NSE的诊断价值最高。结论多种血清标志物检测在肺癌的辅助诊断中均有一定的价值,均可作为肺癌辅助诊断的参考指标。

Objective To investigate the value of tissue polypeptide specific antigen（ TPS）,neuron specific enolization enzyme（ NSE）,carcinoembryonic antigen（ CEA）,carbohydrate antigen 19-9（ CA19-9）and cytokeratin fragmet antigen21-1（ CYFRA21-1） in diagnosis of lung cancer,and provide a reference for clinical diagnosis of lung cancer. Methods A total of 100 patients with lung cancer（ group lung cancer） and100 healthy controls（ group control） were included in the present sduty. The levels of serum TPS,NSE,CEA,CA19-9,and CYFRA21-1 were detected dy electrochemiluminescence immunoassay, and the diagnostic sensitivity,specificity,accuracy and diagnostic value of NSE,CEA,CA19-9,CYFRA21-1 and TPS were analyzed. Results The levels of serum TPS,NSE,CEA,CA19-9 and CYFRA21-1 in group lung cancer[（ 166.23±81.62） U/L,（ 11.91±9.32） ng/ml,（ 10.31±6.36） ng/L,（ 16.19±6.71） U/L,2.61±1.21） ng/L,respectively] were significantly higher than those in group control [（ 51.16 ± 7. 12） U / L,（ 7. 31 ± 6. 22） ng / ml,（ 1.66± 1.01） ng/L,（ 2.28± 1.21） U/L,（ 0.36± 0.26） ng/L,respectively]（ all P〈0.01）. In the clinical diagnosis of lung cancer,the sensitivity of TPS was the highest（ 80. 0%）,and the specificity（ 96. 0%） and accuracy（ 73.0%） of NSE was the highest. The TPS,NSE,CEA and CYFRA21-1 has diagnostic significance on lung cancer,and the diagnosis value of NSE was the highest. Conclusion All kinds of serum markers in the diagnosis of lung cancer has a certain value,can be used as a reference index for lung cancer diagnosis.
