摘要
目的初步探讨肥胖与高血压的关系,并测定肥胖型高血压患病者血清脂联素(APN)以及细胞因子TNF-α、IL-6水平。方法选取原发性高血压确诊患者251例为观察组,另选取正常人157例为对照组,对2组临床情况进行调查,测量其体质量、身高及血压,并检测其血中APN、IL-6和TNF-α的浓度。结果在251例高血压患病者中,正常体质量、超重和肥胖者所占比率分别是29.5%、35.2%和35.3%,而在157例健康对照者中,正常体质量、超重和肥胖者所占比率分别是58.6%、29.5%和15.9%。与健康对照相比,高血压患者的IL-6、TNF-α出现明显升高,血清脂联素则呈显著下降(P<0.05)。在102例高血压患者中,超重和肥胖患者与正常体质量的患者相比,其血清中脂联素浓度呈现显著下降(P<0.05),而TNF-α和IL-6无显著性差异(均P>0.5)。结论超重和肥胖与高血压患病有关,此类人群患高血压危险增加。血清脂联素水平降低及细胞因子TNF-α、IL-6的升高与高血压的发病具有一定相关性,具体机制有待进一步观察。
Objective To investigate the relationship between obesity and hypertension , and determine the levels of cytokines TNF-α, IL-6 and serum adiponectin （ APN） in obese patients with hypertension .Methods 251 cases of hyper-tensive patients and 157 cases normal controls were investigated , and their weight , height and blood pressure were measured , and concentrations of APN , IL-6 and TNF-αwere detected in blood .Results In 251 cases of hypertension , the percent-age of normal weight , overweight and obese is 29.5%, 35.2%and 35.3%, respectively , while in 157 cases of normal con-trols, the percentage of normal weight , overweight and obesity were 58.5%, 25.2%and 16.3%.Compared with normal con -trols, IL-6, TNF-αof patients with hypertension appeared significantly increased and the serum adiponectin was signifi -cantly decreased （ P〈0.05） . In 102 cases of hypertension , the serum adiponectin concentration showed a significant decrease （ P〈0.05 ） , and TNF-αIL-6 and no significant differences （ P〉0.5 ） in overweight and obese patients compared with normal-weight patients .Conclusion Overweight and obesity associated with hypertension , and there was an increased risk of hypertension in this population .The decrease of serum cytokines TNF -α, IL-6 and increased adiponectin has certain correlation with the onset of hypertension , the specific mechanism needs further experimental exploration .
