摘要
目的探讨尿微量白蛋白/肌酐(UACR)与2型糖尿病(T2DM)患者视网膜病变(DR)之间的相关性。方法回顾性分析医院收治的256例T2DM患者的临床资料,重点分析其UACR检查结果与眼底摄片资料,并根据UACR水平将256例患者分为尿蛋白正常组155例(UACR水平<30mg/g)、微量尿蛋白组101例(UACR水平30~300mg/g),比较2组患者视网膜病变发生率,总结二者之间的相关性。结果微量尿蛋白组患者平均年龄(56.1±2.1)岁、平均病程(12.5±2.9)年、BMI指数(25.5±1.9)kg/m^2、收缩压(149.5±8.5)mm Hg、2h FPG(15.8±2.1)mmol/L等指标明显高于尿蛋白正常组,差异有统计学意义(P<0.01)。微量尿蛋白组UACR水平(215.2±12.9)mg/g,DR发生率55.45%,均明显高于尿蛋白正常组,差异有统计学意义(P<0.05)。结论 UACR水平高的T2DM患者的视网膜病变发生率偏高,对于高龄、病程长、血压高类T2DM患者要加强监测尿白蛋白及肌酐,及早发现微血管病变,及早预防视网膜病变的发生。
Objective To investigate correlation analysis of urine micro albumin /urine creatinine ratio and type 2 diabetic retinopathy.Methods Clinical data of 256 cases of T2DM patients in our hospital were reviewed , focuses on the a-nalysis of the UACR examination and fundus photo piece of information , and on the basis of UACR levels will 256 cases of pa-tients were randomly divided for proteinuria normal group of 155 cases （UACR levels in 30g/mg below） and urinary micro pro-tein group 101 cases （ UACR level referrals to 30~300 mg/g） , compared two groups of patients with retinopathy incidence and concluded the correlation.Results Microalbuminuria group with mean age （56.1 ±2.1） years old, the average duration of illness （12.5 ±2.9） years, BMI （25.5 ±1.9）kg /m2, systolic blood pressure levels （149.5 ±8.5） mmHg, 2hFPG （15.8 ±2.1） mmol /L and other index was significantly higher than that in normal group （P〈0.01）; trace group UACR levels （215.2 ＋12.9 ） there was significantly higher than that in normal group , and the incidence of Dr 55.45%were signif-icantly higher than those in normal group （ P〈0.01 ）.Conclusion UACR levels in patients with type 2 diabetes retinopathy occurence rate is high, for old age, long course of disease, blood pressure high T2DM patients to strengthen monitoring of uri-nary albumin and creatinine , early detection of microvascular disease , early prevention of the occurrence of retinopathy .
