床旁即时检验在重症创伤患者围术期凝血功能监测中的应用

Research Progress of Perioperative Blood Coagulation Monitoring with Point-of-care Technique in Severe Trauma Patients

重症创伤患者往往伤情复杂,病情较重,可能引起大出血,大量输血可能导致凝血功能紊乱等并发症。床旁即时检验包括血小板功能和血栓弹力图检验,可以迅速地监测凝血功能动态变化,从而指导精确输注各种血制品,纠正凝血障碍,减少盲目大量输血和相关并发症。本文对床旁即时检验在重症创伤外科患者围术期的应用进展,及与传统凝血功能监测相比在早期诊断和减少死亡率等方面的优势进行文献综述。

Severe trauma patients are usually complicated to deal with. Massive hemorrhage or blood transfusion may lead to following coagulopathy. Point-of-care technique includes platelets function test and thromboelastography. The technique can provide rapid evaluation of blood coagulation status,and then deliver specific transfusion therapy in order to avoid excess transfusion and complications. We reviewed the research progress of perioperative blood coagulation monitoring with point-of-care technique in severe trauma patients,and compared it with traditional coagulation test.
