摘要
腹腔镜胰体尾切除(laparoscopic distal pancreatectomy,LDP)术后胰漏是困扰普外科医生的一个难题,LDP术后胰漏预防和治疗的关键在于患者围手术期管理,遵循个体化治疗原则:根据患者胰腺具体情况,选择合理的残端处理方法,术后密切观察引流情况,早期诊断胰漏,进行充分引流;合理应用抗生素,预防感染,避免进一步发生胰周脓肿、出血等严重并发症。
Postoperative pancreatic fistula after laparoscopic distal pancreatectomy is a problem that has plagued surgeon. So far,the key to preventing and managing postoperative pancreatic fistula after laparoscopic distal pancreatectomy lies in good perioperative management,personalized treatment by selecting suitable way to close the pancreatic remnant in line with pancreatic texture and thickness,early diagnosis of postoperative fistula according to careful observation of postoperative drainage,and rational use of antibiotics to avoid the complication of hemorrhage and parapancreatic abscess.
