地塞米松耳后注射治疗突发性聋的临床观察

The effects of postauricular dexamethasone injection for sudden hearing loss

目的探讨耳后注射地塞米松作为突发性耳聋初始激素治疗的疗效。方法 64例突发性耳聋患者随机分为两组,耳后注射组32例(耳),给予地塞米松0.5ml耳后乳突骨膜下注射,每3 d给药1次;静脉用药组32例(耳),给予地塞米松10 mg静脉滴注,每日1次,3 d后减量至5 mg,继续用药3 d后停药。结果耳耳后注射组痊愈15耳,显效5耳,有效3耳,无效9耳,总有效率71.9%;静脉用药组痊愈13耳,显效4耳,有效4耳,无效11耳,总有效率65.6%。两组差异无统计学意义(P>0.05)。结论耳后注射皮质类固醇激素治疗突发性耳聋疗效可靠、操作简单,可作为突发性耳聋激素治疗的初始给药方法,亦可作为静脉应用激素有禁忌时的有效替代疗法。

Objective To investigate the therapeutic effect of postaurical injection of dexamethasone for sudden hearing loss.Methods 64 cases of sudden sensorineural hearing loss were randomly divided into two groups. Postauricular injection group were given subcutaneous injection of dexamethasone 0.5ml once every 3 days. Intravenous medication group received intravenous infusion of dexamethasone 10 mg once a day for three days, then 5mg daily for another 3 days. Results In postauricular injection group, 15 ears cured, 5 ears markedly effective, 3 ears effective, 9 ears ineffctive. The total effective rate was 71.9%. While in the intravenous medication group, 13 ears cured,4 ears markedly effective, 4 ears effective, 11 ears ineffective, and the total effective rate was 65.6%.There was no significant difference between the two groups. Conclusions Postauricular injection of corticosteroids can be used as a preliminary therapy for sudden sensorineural hearing loss.
