乳腺导管原位癌的钼靶X线征象与病理分型研究

Research on Molybdenum Target X-ray Signs and Pathological Classification of Ductal Carcinoma in Situ

目的:分析探讨乳腺导管原位癌的钼靶X线征象和病理分型之间的相互关联为临床治疗方案提供更多参考依据。方法:选取29例于2014年1月至2015年1月在我院就诊的乳腺导管原位癌患者作为研究对象,所有患者均常规进行乳腺钼靶X线检查,比较影像学表现和病理分型、分级之间的关联性。结果:经过研究发现,本组患者中簇状微钙化、钙化斑块影、单纯软组织影、乳头内陷伴乳头后大导管增粗、钼靶阴性例数分别为20例、3例、4例、1例和1例。23例钙化患者呈V型、圆形、不规则形、小簇钙化状例数分别为7例、8例、5例、3例。乳腺导管原位癌粉刺型高核级以线样分枝状钙化为主,非粉刺型低核级以颗粒点状钙化为主,两组比较差异具有统计学意义(P<0.05)。结论:乳腺导管原位癌的钼靶X线表现具有一定的特征性,其和分子生物改变为基础的病理分型、分级存在紧密联系,能够为临床选取治疗方案提供依据,值得临床应用。

Objective： To analyze the correlation between molybdenum target X-ray signs and pathological classification research of ductal carcinoma in situ. Methods： 29 cases with ductal carcinoma in situ,which admitted in our hospital from January 2014 to January 2015,were selected as the research object according to random number table method. All patients were given normal mammary gland molybdenum target X-ray,correlation between imaging findings and pathological classification and grading were compared. Results： The statistical analysis showed that clustered microcalcificafion,calcified plaque,pure soft tissue shadow,nipple invagination with big catheter after great-grandfather,molybdenum target negative in all patients were 20 cases,3 cases,4 cases,1 case and 1 case respectively. 23 patients with calcification of V type,round and irregular shape,small calcification were respectively 7 cases,8 cases,5 cases and 3 cases. Ductal carcinoma in situ acne type high nuclear grade was given priority to with line sample branching calcification,non acne low nuclear grade was given priority to with particle pointlike calcification,the difference was significant statistically significant（ P〈0. 05）. Conclusion： Molybdenum target X-ray manifestation of ductal carcinoma in situ has certain characteristic,and molecular biological changes based on the pathological classification and grading is closely,can provide the basis for selection of clinical treatment,which is worthy of clinical application.
