应用经会阴三维超声在评价选择性剖宫产后盆底功能变化中的作用 被引量:25

摘要 目的探讨应用经会阴三维超声评价选择性剖宫产后产妇的盆底功能变化。方法选取行选择性剖宫产分娩的初产妇86例作为研究对象,于产后42 d行经会阴三维超声检查,观察静息状态及Valsalva动作时有无盆底组织异常,测量并计算膀胱颈移动度、肛提肌裂孔面积等指标。根据超声检查结果,将病例分为有盆底功能障碍组(观察组)以及无盆底功能障碍组(对照组)两组,对两组产妇的临床检查及超声检查结果进行统计学分析。结果 86例选择性剖宫产后产妇中有盆底功能障碍者(观察组)18例,无盆底功能障碍者(对照组)68例。观察组初产妇临床检查压力性尿失禁6例,阴道前壁脱垂1例,子宫脱垂1例,其余10例未检出异常。对照组初产妇临床检查压力性尿失禁9例,其余59例未检出异常。观察组初产妇经会阴三维超声检查膀胱颈活动度增加13例,阴道前壁脱垂2例,子宫脱垂2例,直肠膨出1例。对照组初产妇经会阴三维超声未检出异常。观察组初产妇在Valsalva动作时,膀胱颈移动度明显大于对照组(P<0.05),肛提肌裂孔的面积明显大于对照组(P<0.05),差异有统计学意义。结论经会阴三维超声检查可以早期发现轻度的盆底功能障碍,可以简便有效地评价选择性剖宫产后的盆底功能变化。
