脑小血管病影像学分型与血管性认知功能损害及相关危险因素 被引量:29

摘要 目的通过对脑小血管疾病(CSVD)患者的影像学表现与血管性认知功能障碍(VCI)及相关危险因素进行分析,为临床制定脑小血管性认知功能损害(CSVCI)预测诊断与防治提供依据。方法收集及分析入院颅脑MRI诊断筛选出的CSVD患者163例,根据蒙特利尔认知评估(Mo CA)量表评分<26分为认知功能障碍(CI),将163例CSVD患者分为认知功能障碍组(CI组,n=82)和无认知功能障碍组(n CI组,n=81)。整理分析患者MRI表现和各项相关危险因素与VCI的相关性。结果两组患者的性别、年龄、身高、体重和有无吸烟史、高血压、高三酰甘油血症、2型糖尿病与VCI的相关性无统计学意义(P>0.05),但患者有无高胆固醇血症与VCI的相关性有统计学意义(P=0.021);CI组腔隙性脑梗死病灶计数多于n CI组,差异有统计学意义(P=0.041);CI组白质病变评分高于n CI组,但差异无统计学意义(P=0.309)。结论高胆固醇血症与CSVCI关系密切;腔隙性脑梗死病灶计数可作为CSVCI的预测指标,通过及时采取有效防治措施,降低CSVCI的发生率。
