无创呼吸机对慢性阻塞性肺疾病合并呼吸衰竭的临床治疗效果 被引量:8

摘要 目的:探讨无创呼吸机对慢性阻塞性肺疾病合并呼吸衰竭的治疗效果。方法:选取2012年8月至2015年8月我院收治的115例慢性阻塞性肺疾病合并型呼吸衰竭患者,随机分为治疗组(58例)和对照组(57例)。两组病人均采用抗感染、舒张支气管、化痰、营养支持、纠正水电解质紊乱和使用呼吸兴奋剂等药物的综合治疗。在综合治疗的基础上,对照组给予常规吸氧治疗,而治疗组进行无创呼吸机治疗。观察治疗前后的血气分析结果和临床疗效。结果:两组患者治疗后PaCO_2和PaO_2较治疗前均得到改善(P<0.05),而治疗组改善更为明显,与对照组比较,差别具有统计学意义(P<0.05)。治疗组和对照组的有效率为93.1%和78.9%(P<0.05)。结论:无创呼吸机治疗能较快升高PaO_2,降低PaCO_2,减轻临床症状。 Objective: To explore the clinical effect of noninvasive ventilator on chronic obstructive pulmonary disease complicatedwith respiratory failure. Methods: One hundred and fifteen patients with chronic obstructive pulmonary disease complicated with re- spiratory failure from August, 2012 to August, 2015 in our hospital were randomly divided into treatment group (58 eases) and con- trol group (57 cases). The two groups were treated with comprehensive therapy including anti-infection, expanding bronchus, resol- ving phlegm, nutritional support, correcting the disorder of water and electrolyte and respiratory stimulants. Under comprehensive therapy, the control group was treated with conventional oxygen therapy, and the treatment group with noninvasive ventilator. The results of blood gas analysis before and after treatment and the effective rate were observed. Results: PaCOz and PaO2 of treatment group and control group were improved before and after treatment and those of treatment group were more obviously improved than that of control group (P〈0. 05). The effect rat of treatment group and control group were 93.1% and 78.9%(P〈0.05). Conclusion: Noninvasive ventilator can obviously increase PaO2 and decrease PaCO2, decrease the clinical symptoms and shorten the treatment length.
