应用即时通讯技术对慢性阻塞性肺疾病患者外延式个体化护理模式的建立与实施

The influence of individualized extended care by synchronous communication tools on quality of life of patients with chronic obstructive pulmonary disease

目的探讨应用即时通讯技术建立外延式个体化护理对慢性阻塞性肺疾病(COPD)患者的生活质量的影响。方法将60例COPD患者随机分为干预组和对照组各30例,对患者情况进行调查分类,对干预组通过建立微信平台对患者宣教及生活指导建立一对一外延式个体护理,对照组不实施微信平台式干预,观察2组COPD患者的血气值[血pH值、氧分压(PaO_2)、二氧化碳分压(PaCO_2)]、肺功能[用力肺活量(FVC)、1s用力呼出量/FEV(FEV1%)]的影响,同时比较2组COPD患者急性发作次数、住院时间、医疗费用。结果干预组pH值与PaO_2较对照组明显升高,PaCO_2明显降低,肺功能明显改善,差异有统计学意义(P<0.05);同时干预组总急性发作次数及住院时间、医疗费用均显著低于对照组,差异有统计学意义(P<0.05)。结论即时通讯技术外延式护理干预能有效提高COPD患者的生活质量,且可以降低急性发作次数并减少住院时间,节省治疗费用。

Objective To explore the effects of individualized extended care by synchronous communication tools on the quality of life of patients with chronic obstructive pulmonary disease（COPD）.Methods A total of 60 patients with COPD were assigned and were randomly divided into the control group and the intervention group by random number table.The intervention group were established the one-to-one individualized extended care by WeChat platform,while the control group were not.Then the blood pH value,oxygen partial pressure（PaO2）and CO2 partial pressure（PaCO2）of two groups were compared,and we also compared the times of acute attack,length of hospital stay,costs and prognosis（death rate）.Results The blood pH value and oxygen partial pressure（PaO2）increased obviously while the CO2 partial pressure（PaCO2）decreased obviously,too.The lung function improved significantly in the intervention group.And the times of acute attack,length of hospital stay and costs were also significantly reduced in the intervention group.Conclusion Individualized extended care by synchronous communication tools can effectively improve the life quality of COPD patients,and it can reduce the times of acute attack,length of hospital stay and costs.
