协同护理模式在肾移植术后的应用

Application of Collaborative Care Model After Transplantation

目的:探讨协同护理模式对中国公民逝世后器官捐献(CDCD)肾移植患者对疾病的认知水平、自我护理能力、生活质量以及长期存活率的影响。方法:选取肾移植患者100例(来自50对CDCD供肾),其中每对CDCD的一个供肾移植患者随机分至A组,另一个供肾移植患者分至B组,两组采取不同处理:A组为对照组(n=50),实施常规护理。B组为干预组(n=50),即在常规护理的基础上实施协同护理模式,干预6个月后,采用疾病不确定感量表(MuIs)、自我护理能力测定量表(ESCA)两个方面比较两组干预效果。结果:应用协同护理模式能够显著提高肾移植术后患者的自我护理能力和生活质量,帮助患者改善不良情绪,以及提高对疾病的认知(P<0.05)。结论:协同护理模式能提高肾移植患者对疾病的认知水平,改善负面情绪,提高患者自理能力及生活质量,并对提高移植术后长期存活率有重要意义。

Objective：To study the effects of collaborative care model of the cognitive level,self-care capability,life quality and long-term survival rate in the renal transplantation patients.Methods：One hundred renal-transplantation patients were paired and divided into two groups for conventional nursing（group A）and collaborative caring（group B）.After 50 pairs of patients are defined,group A treats with usual care implementation and group B deals with Collaborative care implementation.After 6month＇s treatments,MuIs and ESCA were used to compare the effects.Results：Implementation of collaborative care could highly improve patients＇ self-care capability,life quality,emotion and the cognition of disease.Conclusion：Collaborative care has significance for improving patients＇ self-care capability,life quality,the cognition of disease and long-term survival rate.
