摘要
目的探讨多学科教育团队干预模式在非内分泌科住院糖尿病患者中的应用效果。方法选取湖南省某三甲医院非内分泌科的住院糖尿病患者117例,通过组建以患者为中心的糖尿病多学科教育团队,包括内分泌科医生、糖尿病专科护士、营养师、糖尿病联络护士等,为非内分泌科住院的糖尿病患者制定规范的治疗方案、配置合理营养餐、制定从入院到出院个体化教育路径等干预措施,比较干预前后患者的糖尿病相关知识、自我管理行为及生活质量。结果通过多学科教育团队对患者在住院期间的全面管理,患者的糖尿病相关知识、自我管理行为及生活质量较干预前均显著提高(P<0.05)。结论多学科教育团队的建立与实施,能有效提高非内分泌科住院糖尿病患者的糖尿病相关知识、自我管理行为及生活质量。
Objective Evaluating the application and effect of the multi-disciplinary education team intervention model in the non department of endocrinology patients with diabetes.Methods To select 117 patients with diabetes in non department of endocrinology of a hospital in Hunan province, through the establishment of the patient as the center of the diabetes education team, including the department of endocrinology doctor, diabetes specialist nurse, a nurse, the diabetes liaison nurse and so on, to establish a standardized treatment plan for the patients with nondepartment of endocrinology, and to establish a reasonable nutritional meal, to make the intervention measures such as the path of individualized education from admission to discharge. And then, comparing the diabetes related knowledge, self-management behavior and quality of life of the patients before and after the intervention.Results Through the comprehensive management of the patients in the hospital during the multi-discipline education team, diabetes related knowledge, self-management behavior and quality of life were significantly improved after the intervention compared with before(P〈0.05).Conclusion The establishment and implementation of the multi-discipline education team can effectively improve the knowledge of diabetes related knowledge, self-management behavior and quality of life of patients with non department of endocrinology in hospital.
