摘要 目的 探讨参麦注射液联合丙氨酰谷氨酰胺对重症急性胰腺炎细胞因子、HSP和HMGB1表达的影响.方法 将84例重症急性胰腺炎患者随机分为研究组(n=42)与对照组(n=42).两组均常规治疗,对照组采用丙氨酰谷氨酰胺治疗,研究组在上述基础上给予参麦注射液治疗.两组疗程均为10d.对比分析两组治疗后血清细胞因子、HSP和HMGB1表达水平变化及用药副反应.结果 两组治疗后血清IL-6、IL-8、TNF-α水平低于治疗前,IL-10水平高于治疗前(P<0.05);研究组治疗后血清IL-6、IL-8、TNF-α水平低于对照组,IL-10水平高于对照组(P<0.05);两组治疗后血清HSP和HMGB1水平低于治疗前(P<0.05);研究组治疗后血清HSP和HMGB1水平低于对照组(P<0.05);研究组总有效率高于对照组(P<0.05);两组用药期间均未出现严重药物副反应.结论 参麦注射液联合丙氨酰谷氨酰胺治疗重症急性胰腺炎效果明显,可降低患者血清IL-6、IL-8、TNF-α水平,升高IL-10水平,改善患者炎症状态,降低HSP和HMGB1表达. Objective To investigate the effect of Shenmai injection combined with alanyl-glutamine on the expressions of cytokines,HSP,and HMGB1 in patients with severe acute pancreatitis.Methods 84 patients with severe acute pancreatitis were randomly divided into a study group (n=42) and a control group (n=42).Both groups were treated with conventional therapy;the control group were treated with glutamine;in addition,the study group were treated with Shenmai injection.The treatment course was 10 d in both groups.The changes of serum cytokine,HSP,and HMGB1 levels after the treatment were compared between the two groups.The drug side effects were analyzed.Results The serum levels of IL-6,IL-8,TNF-α,HSP,and HMGBI were lower and the level of IL-10 was higher after than before the treatment in both groups and in the control group than in the study group after the treatment (P〈0.05).The total effective rate of the study group was higher than that of the control group (P〈0.05).No serious adverse drug reactions occurred in both groups during the period of drug use.Conclusions Shenmai injection combined with alanyl-glutarnine in the treatment of severe acute panereatitis (SAP) is significantly effective,can reduce the patients' serum levels of IL-6,IL-8,and TNF-α,increase the serum level of IL-10,improve the patients' inflammatory state,and decrease the expressions of HSP and HMGB1.
